The documentation from the City of Charlotte was uploaded in place of the NOT Certification Form. The NOT
Cert. Form is a separate PDF that can be accessed at: https:Hdeq.nc.gov/about/divisions/energy-mineral-and-
land-resources/stormwater/stormwater-program/npdes-construction-program. Scroll down to the "How to
Terminate a Certificate of Coverage" section and the form is the second hyperlink.
Additionally, Freedom Opportunity Fund, LLC is the permittee/legally responsible entity, so Nick Satey or
another responsible officer must sign the NOT Cert. Form and the electronic NOT request. Mr. Larach cannot
sign because Choate Construction is not the permitted entity.
Please submit a new NOT request once all documentation is in order. Thank you.
