Dear Mr. Hall:
This letter is in reference to your permit applications received on May 11, 2021 for Phase 3 (Version C)
and October 5, 2021 for Phase 4 (Version D) for the construction and operation upon certification of the
subject wastewater collection system.
Pursuant to multiple additional information requests via emails on July 91", July 215t, and September 23rd
as well as, a status inquiry on November 30, 2021 after receipt of Phase 4 application, you were asked by
Bryan Lievre of the Wilmington Regional Office, to provide additional information. As of today, this
information has not been received, therefore, the Division of Water Resources must return your
applications as incomplete in accordance with North Carolina General Statute §143-215.1.
Please be advised that construction and/or operation of wastewater collection system extension without
a valid permit is a violation of North Carolina General Statute §143-215.1 and may subject the
owner/operator to appropriate enforcement actions in accordance with North Carolina General Statute
§143-215.6A-6C. Civil penalties of up to $25,000 per day per violation may be assessed for failure to
secure a permit required by North Carolina General Statute §143-215.1.
When you have obtained the requested information that is needed to make your application package
complete, you may submit a new application package using the most current application materials and
appropriate permit processing fee.
