Bridgeton, NC Craven County Permit Number WQ0002503 Notice of Violation and 8/5/2016 Compliance Inspection NOV-2016-LV-0477 Dear Mr. May: VIA EMAIL & CERTIFIED MAIL On behalf of Frit Car, Inc., in the matter referenced above, please allow the following to serve as an update of on -going assessment and monitoring activities in regards to the above reference Notice of Violation (NOV). Please find below a recap of the initial findings, activities completed to date per the initial findings, current findings and recommendations. Initial Findings of CFM (POLY) Based on review of the systems referenced above, CFM (POLY) would like to offer the following findings: 1. CFM (POLY) prepared potentiometric contour maps based on the data collected during past sampling events. I have attached two of the potentiometric maps from two sampling events roughly a year apart (March 27, 2015 & March 22, 2016). These potentiometric maps indicate that the general direction of groundwater flow is to the north-northwest. Based on these observations, monitoring well MW2 is the up -gradient well in the network, monitoring wells MW1 and MW3 are cross gradient wells in the network, and monitoring well MW4 is the down -gradient well in the network. The groundwater flow direction is consistent across all sampling events evaluated (last two years). Potentiometric maps for the above referenced sampling dates are included as attachments to this document. The individual sampling event analytical results are placed adjacent to the monitoring well locations on the figures provided. 2. CFM (POLY) reviewed the sampling data referenced in the Notice of Violation (NOV). The field data collected by the laboratory during sampling indicates that all samples that were collected had a degree of turbidity which was described as color. No turbidity measurements were made in the field. 3. CFM (POLY) examined the wells in the network using disposable bailers to evaluate the appearance of the well contents after purging. CFM (POLY) sampled MW1 for bench scale laboratory testing. The unpreserved sampled was shipped to our office for further evaluation. To mimic a sample collected from an adequately developed well utilizing a low flow/low stress sampling method, the sample was allowed to settle for approximately 2 hours. After settling, a sub -sample was removed from the supernant liquid. The sub -sample was less than 10 NTUs. The sample was preserved to a pH < 2.0 with HNO3 and submitted to the laboratory for bench -scale analysis. The results of the analysis for Iron (Fe) and Chromium (Cr) were 0.776 mg/I and <0.100 mg/I, respectively. For comparison, the March 26, 2016 Fe sample result for MW1 was 12.8 mg/I. The analytical detection limit used by the laboratory was not accurate enough to provide a comparison for the Chromium result. All future analysis will be performed using ICP. A copy of the analytical results from the bench -scale testing is included as an attachment. 5. Three of the monitoring wells in the network were installed in July of 1986. Danny Hornbeck of Frit Car was employed by the facility during that period and recalls the screen material being metal. There is no definitive information available defining the type of screen or the construction techniques used. As recommended in the previous update, Frit Car Inc. has completed the following: 1. Frit Car redeveloped the existing wells in an effort to improve the sample collection quality. The well redevelopment followed the APS Metals Policy'. 2. The effort to rehabilitate MW 3 was not successful. Monitoring wells MWS, MW6 and MW7 have been installed. MW 5 was installed approximately 50 feet north of MW4 but within the property setback (80 feet) and within approximately 80 feet (<250 ft) of the spray field. MW 6 was placed adjacent to MW 3 for a replacement well. MW 7 was installed approximately 60 feet south and west of MW2. All new well installation and their locations followed 15A NCAC 2L .0107(c) for placement within prescribed boundaries. The well locations and casing elevations were all surveyed. The survey coordinates are provided as Attachment A. 3. Frit Car began low flow or quiescent (low stress) sampling techniques to obtain samples. The wells were purged with a peristaltic pump and routed through a Horiba multi -parameter flow cell. The purging continued until the temperature, dissolved oxygen, conductivity and turbidity reached a steady state. Once all the purge parameters reached equilibrium, the sample was taken. Additional sampling of the monitoring wells will be completed as part of the on -going compliance activities. All results of the low flow sampling results are presented herewith to the Department. 4. Frit Car drained and cleaned the existing treated wastewater tank. This task was completed on August 10, 2017. Current Findings The results of the low flow sampling have resolved the majority of the non -compliances. The exceptions to that are exceedances in both chromium and iron. The sampling data is presented in Table 1 below. Chromium exceeded the old permit limit of 0.01 mg/I during the third tri-annual event of 2016 for MW2 and MW4. That event was directly after the initial re -development of the existing wells. It is assumed that is a typo in the permit and should be listed as 10 ug/I or 0.01 mg/I. The iron exceedances are more frequent but are limited to the hydraulically up and cross -gradient locations. Please see potentiometric surface maps in Attachment B. The iron permit limit was exceeded in all three events in CY2017 in MW2 and MW7 (new), in the 1st and 3rd events in MW5 (new), and in the 2nd and 3rd events in MW6 (new). MW2 has historically been located in a hydraulically up -gradient position. MW6 (new) that is slated as a replacement for MW3, and, is the hydraulically up -gradient well in the 2"d and 3rd sampling events of CY2017. MW's 5 & 7 are hydraulically cross -gradient. The two wells in the system that always remain in the hydraulically down -gradient position are MW1 and MW4. No exceedances were noted in either down -gradient well during the last four low -flow sampling events. Recommendations The iron and chromium exceedances are potentially the product of background metals in the soil. To evaluate the metal concentrations in the background soils, Frit Car proposes to take background soil samples and analyze them for the presence of chromium and iron. This task will be completed within the next 30 days. If the data indicates that the soil is a contributor to the exceedances and not the land application of wastewater, Frit Car will request that the background values be taken into consideration in how the permit limits are complied with. In addition, Frit Car will add the testing of iron and chromium to the suite of analysis performed on the wastewater being transferred to the spray field. If the exceedances are a product of the application of wastewater and not elevated background conditions and thus compliance with 15A NCAC 2L .0202 will not be achieved, Frit Car will complete the full assessment of the spray field per 15A NCAC 2L .0107, .0108 and .0106(d)(1) within 90 days after the background sampling and analysis is completed, if necessary. Once these issues are resolved, Frit Car will request a modification to the existing permit WQ0002503 to include the additional wastewater sampling as well as adding MWs 5, 6 & 7 to the monitoring system in the permit. Bridgeton, NC Craven County Permit Number WQ0002503 Notice of Violation and 8/5/2016 Compliance Inspection NOV-2016-LV-0477 Dear Mr. May: VIA EMAIL & CERTIFIED MAIL On behalf of Frit Car, Inc., in the matter referenced above, please allow the following to serve as an update of compliance efforts regarding the requirements specified in your Departments letter dated August 25, 2016. CFM conducted a site visit and site review of the wastewater treatment system, spray field and monitoring well network during the week of October 31, 2016. An invitation was extended to Justin Davis and Randy Sipe of your department to meet on site and review the data developed to date. On behalf of Frit Car I would like to thank Mr. Davis and Sipe for their participation. Based on review of the systems referenced above, CFM would like to offer the following findings: 1. CFM prepared potentiometric contour maps based on the data collected during past sampling events. I have attached two of the potentiometric maps from two sampling events roughly a year apart (March 27, 2015 & March 22, 2016). These potentiometric maps indicate that the general direction of groundwater flow is to the north-northwest. Based on these observations, monitoring well MW2 is the up -gradient well in the network, monitoring wells MW1 and MW3 are cross gradient wells in the network, and monitoring well MW4 is the down -gradient well in the network. The groundwater flow direction is consistent across all sampling events evaluated (last two years). Potentiometric maps for the above referenced sampling dates are included as attachments to this document. The individual sampling event analytical results are placed adjacent to the monitoring well locations on the figures provided. 2. CFM reviewed the sampling data referenced in the Notice of Violation (NOV). The field data collected by the laboratory during sampling indicates that all samples that were collected had a degree of turbidity which was described as color. No turbidity measurements were made in the field. 3. CFM examined the wells in the network using disposable bailers to evaluate the appearance of the well contents after purging. Although no field measurements of turbidity were conducted, 1 2135 University Blvd. Suite A 1 Tuscaloosa, AL 35401 1 205.752.4037 1 FX 205.752.40631 www.cfm-grp.com Mr. David May NCDENR Page 2 November 16, 2016 the samples removed after purging indicated a significant amount of turbidity (e.g. based on our examination of the well contents we estimated to be greater than 800 NTUs). 4. CFM sampled MW1 for bench scale laboratory testing. The unpreserved sampled was shipped to our office for further evaluation. To mimic a sample collected from an adequately developed well utilizing a low flow/low stress sampling method, the sample was allowed to settle for approximately 2 hours. After settling, a sub -sample was removed from the supernant liquid. The sub -sample was less than 10 NTUs. The sample was preserved to a pH < 2.0 with HNO3 and submitted to the laboratory for bench -scale analysis. The results of the analysis for Iron (Fe) and Chromium (Cr) were 0.776 mg/I and <0.100 mg/I, respectively. For comparison, the March 26, 2016 Fe sample result for MW1 was 12.8 mg/I. The analytical detection limit used by the laboratory was not accurate enough to provide a comparison for the Chromium result. All future analysis will be performed using ICP. A copy of the analytical results from the bench -scale testing is included as an attachment. 5. Three of the monitoring wells in the network were installed in July of 1986. Danny Hornbeck of Frit Car was employed by the facility during that period and recalls the screen material being metal. There is no definitive information available defining the type of screen or the construction techniques used. Based on the findings above, CFM would like to offer the following opinions and recommendations for the immediate plan of action to address the issues. 1. As stated in Item 1 above, the general groundwater flow direction is north north-west. Permit number WQ0002503, Attachment C details that the concentration of Iron (Fe) and Chromium (Cr) shall not exceed 300 µg/I (0.30 mg/I) and 10 µg/I (0.01 mg/I), respectively. NOV-2016-LV- 0477 details eleven exceedances of the limits established under the permit. Of these eleven exceedances, three of the eleven were identified in the down -gradient well. Eight of the eleven exceedances are identified in either up -gradient (4) or cross -gradient wells (4). Since the highest exceedances of both Fe and Cr have appeared in areas other than the down -gradient location, it is reasonable to assume that either less than ideal sampling techniques and/or elevated background concentrations are the cause of the apparent exceedances. If background values are determined to be the cause of permit exceedances, a plan for altering the existing site conditions via a permit modification will be submitted to the Department per 15A NCAC 2L .0106(d)(1). 2. Frit Car will redevelop the existing wells in an effort to improve the sample collection quality. The well redevelopment will follow the APS Metals Policy'. 3. If the effort to rehabilitate the wells does not prove reliable, Frit car will install new wells to replace the existing wells. Any new well installation and their locations will follow 15A NCAC 2L .0107(c) 4. Frit Car will mandate low flow or quiescent (low stress) sampling techniques to obtain samples which are more representative of actual site conditions (samples with turbidity of less than 10 NTUs). Additional sampling of the monitoring wells will be completed as part of the assessment activities. All results of sampling will be presented to the Department. 5. Frit Car will drain and clean the existing treated wastewater tank. A review of operation of the wastewater treatment plant (pH adjustment and solids removal) will be evaluated to determine 1 NCDENR Memorandum dated May 13, 2013 concerning "Aquifer Protection Section Policy for Metals Determinations Required by Title 15A, North Carolina Administrative Code, Subchapter 2L" Mr. David May NCDENR Page 3 November 16, 2016 if any improvements can be made to increase the efficiency of the system. It is noted that any modification to the system may require approval or permit modification. 6. Once the above items 2 through 5 have been completed, CFM will present a report detailing the findings. If the findings indicate that compliance with 15A NCAC 2L .0202 will not be achieved, Frit Car will immediately begin fulfilling all of the remaining requirements of 15A NCAC 2L .0107, .0108 and .0106(d)(1). The redevelopment, rehabilitation and additional sampling are scheduled to begin on November 29, 2016. To facilitate the above, CFM on behalf of Frit Car Inc., respectfully requests a 90 day extension of time to determine the efficacy of the current monitoring system as well as past/future sampling techniques, potential necessary upgrades of the monitoring system and/or the necessity of continuing assessment or corrective action(s). Permit Number WQ0002503 Craven County Dear Mr. Sipe: At the request of the Washington Regional Office, Frit Car, Inc. has recalculated a site -specific dissolved iron background level using data from monitoring well samples collected from temporary wells TM1 and TM3 during the period May to December 2020. The Department has previously indicated that the proposed method of two sigma plus an average was acceptable for calculation of the iron background level. The calculation for the proposed dissolved iron background level is included in Attachment A. The May to December 2020 iron concentrations table is also included in Attachment A. Frit has also compared these proposed background levels to dissolved iron concentrations at wells along the Review and Compliance Boundaries. The Waste, Review and Compliance boundaries are presented on the figure included as Attachment B. Wells along the Review boundary include MW-1 to the northwest of the Waste boundary; MW-5 northeast of the Waste boundary; and, MW-6 to the southeast of the Waste boundary. MW-7 is located along the Compliance boundary at to the southwest of the Waste boundary. Dissolved iron concentrations were detected at or below the calculated background level at the Review boundary in MW-1, MW-5, and MW-6. During the May to December sampling period, the iron concentration in MW-7 exceeded the proposed background level at the Compliance boundary two of the eight sampling events (June and December), therefore exceeding the proposed background concentrations and thus requiring Frit Car to conduct Corrective Actions per your May 11, 2021 letter. Frit is in the process of evaluating water treatment modifications to reduce the iron concentrations in the effluent to below the proposed calculated background level; therefore, if the dissolved iron present in the effluent applied to the spray fields is less than the calculated background, then any exceedances in the wells on the spray field must be attributed to naturally POLY 1...- 234 Aquarius Drive, Suite 116 www.poly-inc.com Birmingham, Alabama 35209 Fe Background Evaluation-Frit Car Mr. Randy Sipe September 23, 2021 Page 2 occurring background concentrations. To comply with Corrective Action requirements, the effluent will be sampled prior to application. Any effluent that exceeds the background level for dissolved iron will be hauled by licensed transport to an appropriate disposal facility. In reviewing the above information, please note that the facility has not applied any effluent to the sprayfield since February 2019. All groundwater data gathered during the eight -month data development period should not reflect any direct influence of effluent application. The report was prepared in response to an August 25, 2016 Notice of Violation (NOV) issued to Frit Car, Inc. from this office and to satisfy the requirements of 15A NCAC 2L .0106(d). Several items in the report require additional clarification or further investigation. They are as follows: 1. The report presents the groundwater data collected at the facility since 2017; however, no interpretation of the data was offered. If Frit Car, Inc. wishes this office to consider site specific groundwater background concentrations, the report should be revised to propose them accompanied by a discussion supporting the method used to develop the background concentrations. This work should be performed by a North Carolina licensed geologist. 2. Background values for naturally occurring constituents should be developed using samples from areas outside any potential impacts or influence from activities at the subject facility. Based on the potentiometric maps included in the report, the proposed background locations (Temporary wells 1 through 3) were located down gradient of the wastewater spray field. Based on their locations, the subject wells may not allow samples to be collected from areas that would be considered truly representative of background conditions. The report should be expanded to include justification for the background locations. Table 1 shows increases in concentrations of chromium detected in monitoring wells MW I, MW2, and MW3 generally during the time period from 2013 to 2017 which appears to correlate with increases in chromium concentrations in the wastewater effluent during the same time period (see the attached graph). Has Frit Car, Inc. considered this relationship as part of their groundwater evaluation? 5. The January 30, 2018 report from CFM(POLY) stated that Frit Car, Inc, would collect data on the iron concentration in the wastewater effluent as part of their investigation. Permit Number WQ0002503 Craven County Dear Mr. Sipe: via Electronic Mail This letter report presents the groundwater analytical data collected from the spray field wells between May and December 2020 to develop background concentrations for iron as well as a historical table for analysis of the effluent. Groundwater Flow Direction The potentiometric surface elevation maps generated for each of the six sampling events indicated a northwest or west-northwest flow direction for the May, June, August, November and December measuring events. The flow directions for the three remaining months were west- southwest (July), north-northeast (September), and east-northeast (October 2020). During the five months that the flow direction was toward the northwest or west-northwest (toward the facility), Monitoring wells MW2, MW6, and MW7 were the upgradient wells. The other three months indicated different flow directions. The difference in the elevations of the wells is quite small (less than 0.4 feet) across the spray field area. The shallow depths to water and the tidal environment may be contributing to the unpredictable flow directions. Previous discussions indicated that the Department did not want Frit Car (Frit) to use Temporary Well 2 as a background well. POLY has used Temporary Wells 1 and 3 as background wells. Metals Analysis Frit used low -flow sampling methods in sampling of the temporary wells and permanent spray field wells to develop the background values. Frit collected samples for total and dissolved metals for eight consecutive sampling events (May 2020 through December 2020). Both the temporary wells (TW1 & TW3) and permanent monitoring wells were sampled during this period. Total chromium and total iron were historically elevated in previous years sampling events. Frit instituted a low flow sampling methodology during the December 2016 sampling event. Based on these results it is our opinion that the low flow sampling technique will allow the facility to substantially comply with the 0.01 mg/I chromium limit. Therefore, a revision to the current permit limit for chromium will not be requested at this time. The sampling results for total iron indicate compliance cannot be achieved even while utilizing low flow sampling techniques. The iron values in the generally up -gradient wells (MW2, MW6 & MW7) routinely exceed the permit limit of 0.3 mg/I. It is reasonable to assume that if groundwater flowing across the spray field from an upgradient direction exceeds the permit limit, compliance with the permit limit is not achievable. Iron values from the temporary wells TW1 and TW3 routinely exceed or are near the permit limit. The data generated from sampling the existing generally up -gradient monitoring wells (MW2, MW6 & MW7) and temporary wells (TW1 & TW3) for the period covering May 2020 and December 2020 were used to calculate background levels for iron. The Department has previously indicated that the proposed method of two standard deviations plus an average was acceptable for calculation of the iron background level. Bridgeton, Craven County, North Carolina Permit No. WQ0002503 Dear Mr. Davis: The Washington Regional Office has reviewed the results of the Iron background investigation that Poly, Inc. has performed at the Frit Car facility in Bridgeton, NC that you submitted to our office on April 29, 2021. The report proposes the use of background data from temporary background wells TM1 and TM3 as well as monitoring wells MW2, MW6, and MW7 located around the facilities spray field. It was our understanding, based on the proposal submitted to our office in your May 11, 2020 letter, that site specific Iron background levels would be developed from data collected at TM1 and TM3, located in areas of the facility away from the spray field. Our office is not comfortable with using data from wells located in the vicinity of the spray field and inside the Compliance Boundary, as defined in 15A NCAC 02L .0107, to develop background levels. Especially, since effluent quality data at the facility included in the report presents Iron concentrations with an average of approximately 26 mg/L, which is almost one order of magnitude greater that the North Carolina Groundwater Standard of 0.3 mg/L. We request that you recalculate a site specific Iron background level using the data from TM1 and TM3. If Iron concentrations are detected above this background level in wells located at the Review Boundary around the spray field, as defined in 15A NCAC 02L .0108, Frit Car will need to evaluate if the background level has been exceeded at the Compliance Boundary, as required by 15A NCAC 021 .0106(b). Exceedance of the background level at the Compliance Boundary will require development of a Corrective Action Plan, as required by 15 A NCAC 02L .0106(f) and (h). Corrective Actions could include modifications to the wastewater treatment system to reduce the Iron concentration in the effluent or the use of other effluent disposal options.
