DWR received on December 30, 2013 the corrected PCN application reflecting the 66 linear feet of
stream and the agent authorization form. Further review indicates the need for a $240 permitting fee
based on the noted stream impact and the desire for written approval for the GC3885, NW13.

3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
Site is comprised of an existing single - family residence constructed in 1997. Erosion is evident at the rear of the house
which has created destabilization of the rear deck, below patio and potential house footings. Several sink holes were
observed throughout the riparian area. Invasive species such as liriope, mondo grass, and english ivy are also evident.

Stabilize the riparian area immediately south of the existing home.
3e. Describe the overall project in detail, including the type of equipment to be used:
Please refer to the attached Retaining Wall Construction Plans. A temporary road and construction ramp is proposed on
the east side of the lot. This will provide access to the proposed gabion wall construction area. A 3 -tier gabion wall is
proposed along approximately 66- linear feet of stream bank and lower riparian area. A permanent erosion fabric and
vegetation will be installed immediately above the gabion walls. Excavation of the lower riparian area will conducted and
then filled with a reinforced slope backfill area. A double sift fence will be installated at the bottom of the construction
area, immediately above the creek, to prevent erosion. All areas will be stabilized with proper vegetation at the conclusion
of the project.
