Dear Mr. Burdette:
On December 13, 2013 the Division of Water Resources(DWR)received information for the above
referenced project. The DWR has determined that the application was incomplete and/or provided
inaccurate information as discussed below.
Additional Information Requested:
® The correct permitting fee to remit for the above project is$570 based on the stream
impacts over 150 linear feet. Check#2490 for$240 was applied to the project. An
additional$330 is required to begin the revievy and approval process. Please remit to:
DENR/DWR 1650 Mail Service Center, Raleigh NC 27699-1650.
Please submit this information within 30 calendar days of the date of this letter.This letter only
addresses the application review and does not authorize any impacts to wetlands,waters or protected
buffers. Please be aware that any impacts requested within your application are not authorized (at this
