Request Form is used to satisfy 40 C.F.R. Section 121.4(a) which states "At least 30 days prior to

agency." In accordance with 40 C.F.R. Section 121.5(b)(7), and (c)(5), all certification requests must
include documentation that a pre -filing meeting request was submitted to the certifying authority at least 30

The project site is rural -residential, south of US Hwy 321. Primary land use consists of pasture, undeveloped
forest, and residence. The Subject Property is currently used for active cattle grazing. Cove Creek flows west on
the property, just south of US 321 N.

4h. Explain the purpose of the proposed project:
The purpose of the project is to stabilize eroding stream banks on Cove Creek. Lack of a vegetated riparian
buffer, runoff from adjacent roadways, and an increased sediment load due to upstream agricultural land use has
caused degradation on this reach. Streambank benching and sloping, installation of natural channel design
structures, and a planted riparian forest buffer will be used to restore the reach.
4i. Describe the overall project in detail, including the type of equipment to be used:
The project will provide bank stabilization and grade control using natural channel design structures including J-
hooks, crossvanes, rock sills, and revetments (boulder, toewood, rootwad). A bankfull bench will be constructed
and the banks will be sloped back and stabilized using coir matting and live stakes. An existing stormwater
drainage originates up -valley to the south of Cove Creek. Rock -step structures will be installed in the stormwater
drainage to provide grade control during heavy precipitation events and to prevent rilling of newly formed
floodplain bench. A temporary construction crossing will be installed upstream of the existing bridge on -site for
equipment access to stream. Existing bridge cannot hold the weight of heavy machinery. A riparian buffer of
native trees and shrub species will be planted. Livestock will be excluded from stream access using fencing.
Equipment used for this proposed construction includes track -hoes, skid steers and dump trucks.

E. Impact Justification and Mitigation
1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts through project design:
The proposed work and associated impacts are intended to stabilize approximately 2002 In ft of stream bank
along Cove Creek while minimizing impacts to aquatic resources. Design measures which minimize impacts
include installation of natural channel design structures which will improve stream stability and provide habitat.
All disturbed buffer areas will be immediately seeded upon completion, matted and stabilized at the close of
each construction day.
1 b. Specifically describe measures taken to avoid or minimize proposed impacts through construction techniques:
All streambank disturbances will be stabilized same day and covered with coir matting and live staked to reduce
sedimentation during construction. Only well maintained equipment will be used; no leaks will be tolerated.
Equipment will work from the banks whenever possible while performing the work and will only work in the
channel when absolutely necessary. Following construction, the banks of the restored channel will be seeded
with a native floodplain mix (permanent seed). The forested riparian buffer will be planted with a variety of native
trees and shrubs following construction.
