Division of Water Resources' Central and Regional staff has reviewed the application package
received September 15, 2021. However, additional information is required before the review may be
completed. Please address the items on the attached pa ges no later than the close of business on January
10, 2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through F, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.

revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper

E. Land Application Site Certification (FORM: LASC 06-16) Form:
1. The photocopied black and white site maps for all the new and modified fields are not always clear
and legible. Legible electronic versions of these maps shall be provided that are in color and meet
the requirements listed in the "Map Guidance for Residuals Land Application Permits" and "Soil
Scientist Evaluation Policy" memorandums. The only maps that need to be included are the new
and modified fields.
2. Item B of the Instructions for FORM: LASC 06-16 states that a soils evaluation and agronomic
evaluation in accordance with the "Soil Scientist Evaluation Policy" shall be provided for new sites,
and may need to be included for modified sites. These evaluations shall be provided for the new
sites, and shall also be provided for the modified sites to verify that the soils in the new sections
for fields are consistent with the existing parts of the field.
3. In accordance with Item C of the Instructions page, Waterbody and Classifications Information
shall be provided for all new sites, and Section II of the LASC shall be completed in the LASC
with the location and waterbody subbasin information. This section shall be included for fields NC-
AM-10-01 and NC-PS-03 fields 5 & 6.
4. There were no maps or Land Owner Agreements (LOA) provided for fields NC-PS-03 fields 5&6.
Please provide this information if they are to be proposed in this modification.
F. Landowner Agreement (FORM: LOA 06-16) Form:
1. See Comment E.1. The submitted LOA for fields NC-PS-03 do not include fields 5 & 6 referenced
in the LASC.
