Dear Ms. Kornegay:

The Division of Water Resources (DWR) has reviewed your application, received on November 30, 2018, for
coverage under General Permit NCG560062. To enable us to complete our review in accordance with N.C.G.S.
143-215.1 and 15A NCAC 02H .0105, DWR needs additional or revised information to address the following
comments:

According to our database, Duplin County Health Department’s notice of intent (NOI) for coverage to
discharge pesticide products under General Permit NCG560000 was received by this office in November
2018. As a result of an oversight, a certificate of coverage (COC) was not issued. General Permit
NCG560000 is currently in the process of being renewed. In order to update our records regarding this
matter we will need an updated NOI. If you would like to apply for coverage under this renewal for the
general permit, please complete and return the attached notice of intent (NOI).
