The North Carolina Division of Water Resources conducted a Compliance Evaluation
Inspection (CEI) of the subject wastewater treatment facility on December 1, 2021.
This inspection was conducted to verify that the facility is operating in compliance with
the conditions and limitations specified in Certification of Coverage No. NCG551457. The
findings and comments noted during this inspection are provided in the enclosed copy of
the inspection report entitled "Compliance Inspection Report".

Treatment system consist of a Single Family Domestic Wastewater Discharge from a Septic
Tank/Pump Tank to treatment filters. Flows from residence is intermittent, there was no discharge at
time of inspection.
The treatment system appeared in good condition at time of inspecction.
Filter spray system appeared to be working properly with good spray distribution across filter media.
All high water alarms and lights were operational at time of inspection.
Discharge is to ephemeral drainage that flows into (Lake Tillery) Yadkin -Pee Dee River that is
Classified as WS-IV-B;CAwaters.
