Yes, we do need the address to be revised. Is there a form or anything that we need to fill out to request the address
change? I did not find anything on the website to request the revision for the organizational entity address except to
contact you. Please let me know what I need to do to get this corrected.

The Owner Verification report reflects what we have in our permitting database for the Permittee Organization. The
Owner Affiliation (Doug Hillman) does have the address you want, but the Owner/Operator organization is the office
address you pointed out. That's how it is currently set up in our system. It may reflect information that was on the
original application for the permit.
There have been subsequent requests to update Legally Responsible Official/Owner Affiliation, but those are a person
and not the organizational entity. We do not have any records of requests to revise the Organizational entity
information. Does the address need to be revised?

Good Morning Bethany,
I am trying to get our company registered in the e-DMR registration form so that I can get it sent in so I ran our owner
verification report in accordance with step 1 and our owner/permittee address is incorrect. How can I get this
corrected? I have attached the report for reference. It is correct on the permit summary report which is also attached
for reference. It should match the facility address. The address that is listed for the owner on the owner verification
report is a sister division by a different name that is just an office and that is closing by next month. Since we need to
get set up and start reporting in January, can I go ahead and send in the registration form while this is being corrected?
Please kindly let me know so I can get us on set up as I know we are behind on this. Thank you.
