However, additional information is required before the review may be completed. Please address the items on the attached page no later than the close of business on November 5, 2021. Staff Report/Inspection: During our review of the renewal application and subsequent facility inspection, our Regional Office staff noted that a chemical feed was added to the treatment system. Available information on this chemical feed was limited, and our office does not have any records of a minor modification being submitted for review. Changes to the treatment system (including the addition of chemical feeds) require a minor modification of the permit. Please provide information regarding chemical type, chemical dosing, chemical storage, equipment location and sizing, and piping and other appurtenance information related to the chemical feed system. Please submit the application form HRIS 06-16, signed and sealed engineering calculations, plans, and specifications pursuant to 15A NCAC 02T .0704. Our understanding is that this equipment has already been installed, as such please also provide the date it was installed, an executed engineering certification, and the updated Operation and Maintenance (O&M) and Residuals Management Plans.
