I'm putting this project on hold. Please send the additional information when you are able. Let me know if you have any
questions.
On November 5, 2021, the Division of Water Resources (Division) received your application requesting a 401 Water
Quality Certification from the Division for the subject project, with the required fee received on November 15, 2021. The
Division has determined that your application is incomplete and cannot be processed. The application is on -hold until all
of the following information is received:
• Please submit a photo showing the extent of shoreline proposed for stabilization.
• In order for the Agent Authorization form to be valid, both parties need to sign it (the landowner and the agent).
Please sign the agent authorization letter. The agent authorization letter submitted with the application
(attached) can be updated with your signature, or a new form can be signed by both parties. [15A NCAC 02H
.0502(a)(11)]
Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the
proper consideration of the application. Please provide your response by January 5, 2022. If all of the requested
information is not received, the Division will be unable to approve the application and it will be denied as incomplete.
The denial of this project will necessitate reapplication to the Division for approval, including a complete application
package and the appropriate fee.

I need a photo showing the eroding shoreline that is needing stabilization, aside from where the failing seawall will be
replaced.
Also, you need to sign the agent authorization form (both parties need to sign it).
