Dear Mr. Richter:
On December 1, 2021, Brian Lambe from the Wilmington Regional Office of the Division of Energy, Mineral, and
Land Resources (DEMLR), conducted a compliance evaluation inspection Warsaw Oil Processing Facility located
in 376 W Park Drive, Duplin County, North Carolina. The site drains to Turkey Creek, which is currently classified
as Class C, Sw Waters in the Cape Fear River Basin.
The following observations were noted during the DEMLR inspection (please see the attached addendum for
additional information about your permit):
1) Stormwater Pollution Prevention Plan (SPPP)
A Stormwater Pollution Prevention Plan (SPPP) has been developed, recorded, and properly implemented.

Qualitative monitoring has been conducted and recorded in accordance with permit requirements.

Analytical monitoring has been conducted and recorded in accordance with permit requirements.

No recent sampling events have been recorded. Diligent effort needs to occur to obtain samples.
Other Observations:
Wastewater is being discharged to the stormwater pond 1. Install containment to prevent contamination of
stormwater. Provide this office of a timeline when the containment will be installed.
Thank you for your assistance and cooperation during this inspection. Please be advised that violations of the
NPDES Stormwater General Permit may be subject to a civil penalty assessment of up to $25,000.00 per day

Inspection Summary.
Met with Kelly Quinn, EHA Manager/QA Whole Harvest, Warsaw regarding NCG060290.
The SPPP was professionally prepared and updated according to the permit requirements.
Investigation of the site conditions revealed that there are issues with boiler blow down and product waste entering into pond
1. This discharge is wastewater and must be contained, Provide a schedule that is reasonable to establish containment.
The management of the flashboard riser in pond 1 needs to be documented. Currently, the flashboard is halfway up the riser.
The bottom of the riser showed evidence of some discharge. The pond condition appeared to be cloudy. A priority needs to
be placed on obtaining a sample for the discharge.
You may apply for Representative Outfall Status (ROS) for pond 2, represented by pond 1
