The Wilmington Regional Office of -the Division of Energy, Mineral and Land Resources issued a Stormwater Management
Permit Modification 2 for ER & Surgical Services Pavilion on April 5, 2012. This permit modified permit SW8 950317, issued
on August 3, 2006, by revising the permit to increase the built -upon area (BUA) in the high density pocket of an overall 30%
low density development; and by reducing the BUA in the low density section.
However, this permit was a temporary approval pending a modification to expand the wet detention pond in the high density
pocket (drainage area DA1) to handle the actual BUA (pre-88 + newer) plus any future expansion. Presently, for the pocket of
high density, the pond is designed to handle runoff from 176,410 square feet of BUA. For the future modification the pond will
need to be designed to handle 295,594 (pre-1988 + newer) square feet of BUA plus any future BUA expansion. Please
reference the "BUA Summary" table in the permit for a summary of the BUA and project area. ,
In the permit letter, we stated that in order to validate the permit approval, and remain in compliance with the permit
modification, the permittee must submit a new permit modification with revised plans showing future development to DWQ
within 18 months. It has now been thirty (30) months since we issued the permit. However, in light of the letter received from
David Newsom on your behalf, dated July 2, 2014, we are willing to give you an extension for one (1) year. Therefore, please
provide an update by July 2, 2015.
Please be reminded that the Director may modify, revoke and reissue or terminate the permit for cause. In addition, your
current permit is effective from the date of issuance until August 3, 2020, and shall be subject to the conditions and limitations
as specified therein. Please pay special attention to the conditions listed in this permit regarding the Operation and
Maintenance of the BMP(s), recordation of deed restrictions, certification of the BMP's, procedures for changes of ownership,
transferring the permit, and renewing the permit. Failure to establish an adequate system for operation and maintenance of the
stormwater management system, to record deed restrictions, to certify the BMP's, to transfer the permit, or to renew the
permit, will result in future compliance problems.

Dear Mr. Pusey
In response to your email of June 25, 2014, we did indeed once again make contact
with Onslow Memorial Hospital (Mr. Michael Holdren) with regard to current status of
"proposed expansion project" as described in my previous letter to you of April 4,
2012. You will recall that the April 5, 2012 Permit Modification issued by your office
was a "temporary approval' contingent upon expansion of an existing wet detention
pond (DA1) "to handle pre-1988 + newer)" BUA.
Mr. Holdren indicates that the "proposed expansion project' ("courtyard area between
original "main" building and the more recently constructed ER & Surgical Pavilion) is
current on "hold", pending resolution of budgetary issues. Mr. Holdren could provide
no firm dates as to when, and if, these budgetary issues may be resolved.
That being the case, we would request that your office grant the Permittee a
temporary extension of 24 months. It is hoped that these budgetary issues can be
resolved during that period, and we can then provide your office a more "concrete"
assessment of the timeline for expansion of the existing pond.
We are hopeful that this response addresses all concerns.

At present, Hospital really has no firm timeline for the future expansion. Hopefully, in a few months the construction
schedule will be more definitive.
See attached letter. Does this seem reasonable (24 month extension)?
