Staff review of the plans and specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000. We are forwarding Permit No. SW8 900811 Modification dated November 12, 1997, for the construction of Markraft Cabinets. This permit shall be effective from the date of issuance until November 12, 2007, and shall be subject to the conditions and limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this permit. Failure to establish an adequate system for operation and maintenance of the stormwater management system will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands are made this permit shall be final and binding. This permit is effective only with respect to the nature and_volume of stormwater described in the application and other supporting data. 2. This stormwater system has been approved for the management of stormwater runoff as described on page 3 of this permit, the Project Data Sheet. Pond 1 of the stormwater control has been designed to handle the runoff from 86,655 square feet of impervious area; Pond 2 for 3,240 square feet of impervious area. 3. Approved plans and specifications for this project are incorporated by reference and are enforceable parts of the permit. 4. The tract will be limited to the amount of built -upon area indicated on page 3 of this permit, and per approved plans. The following items will require a modification to the permit: a. Any revision to the approved plans, regardless of size. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area. e. Further subdivision of the project area, or construction of built -upon area on any of the other lots owned by J.N. Development as shown on the plans. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. In addition, the Director may determine that other revisions to the project should require a modification to the permit. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. Il. SCHEDULE OF COMPLIANCE The permittee will comply with the following schedule for construction and maintenance of the stormwater management system: a. The stormwater management system shall be constructed in it's entirety, vegetated and operational for it's intended use prior to the construction of any built -upon surface except roads. b. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 2. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: r a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. C. Mowing and revegetation of side slopes. d. Immediate repair of eroded areas. e. Maintenance of side slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of outlet structure, orifice device and catch basins and piping. g. Access to the outlet structure must be available at all times. 3. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ. The records will indicate the date, activity, name of person performing the work and what actions were taken. 4. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. 6. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum of ten years from the date of the completion of construction. 7. Prior to the sale of any portion of the property, an access/maintenance easement to the stormwater facilities shall be granted in favor of the permittee if access to the stormwater facilities will be restricted by the sale of any portion of the property. III. GENERAL CONDITIONS This permit is not transferable. In the,event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143- 215.6C. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 4. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems. 5. The permittee grants DENR Staff permission to enter the property for the purpose of inspecting all components of the permitted stormwater management facility. r 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and reissuance or termination does not stay any permit condition. Permit issued this the 12th day of November, 1997. A preliminary review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. Please relocate the outlet for Pond 2 to opposite the forebay. Short-circuiting of the pond is a concern. 2. The road elevations at the new pavement connection to the existing road indicate. that runoff from approximately 80 linear feet of new road will bypass the pond. The access road detail indicates that cut section ditches are located on each side, but neither the swale location nor the inverts are shown on the plan. 3. FYI -The forebay does not have a minimum surface area requirement of 20% of the required surface area. It only has requirement to hole 20% of the volume of the first inch. 4. From the map provided, showing a subdivision of land into 3 lots, it appears that the new building is situated over the property line. Is this correct? Please make your client aware that using good seed or sod, heavy mulching, immediate repair of eroded areas, and an irrigation system work wonders toward getting and keeping a good grass cover; and that enforcement of this issue will be top priority. 11. No calculations were provided with the submittal, therefore, verification of the pond data cannot be done at this time. A complete review of the pond calculations will be performed when they are submitted. Please note that this request for additional information is in response to a preliminary review. The revised drawing which is dated 12/07/90, shows a detention pond in a different location and with a different configuration than that shown on the earlier plan. In addition, I have made a minor change to the calculations which accompanied the 11/08/90 plan to reflect the different configuration. None of the changes made in the plan or,the calculations changed the required size of the permanent water surface, required storage, water velocities in pipes, pond depths, pipe sizes or any other parameters which would effect the ability of the system to function as originally designed. A 12" diameter pipe was added to carry the drainage from off -site through the bank area of the pond so that the off -site drainage would continue to bypass the detention pond. The pipe will have at least 12" of earth cover and will not interfere in any way with maintenance of the pond. The work covered under this specification consists of installing an unreinforced concrete slab of nominal thickness as indicated on the plans, t- positioning a specifically woven dual wall, synthetic fabric envelope on the slope or surface to be protected and injecting it with fine aggregate concrete. B. The fabric shall be so constructed that placing of the mat is possible without dewatering. 1.02 QUALIFICATION OF SUPPLIER: A. The supplier shall furnish evidence satisfactory to the Engineer, of three years of successful performance in this type of work. B. Throughout the progress of installation of the work included in this section, the supplier shall provide at least one person who shall be familiar with the specified requirements, completely trained and experienced in the necessary skills, and who shall be present at the site and shall direct all work performed under this section. 1.03 SUBMITTALS: A. Submit to the Engineer the following information at least two weeks prior to installation: 1.' Description of materials to be used. 2. Proposed method of installation.. 3. List of material suppliers. 4. Shop drawings indicating individual fabric panels, site location plan and location and details of bulkheads, trenches, laps, etc. 1.04 QUALITY CONTROL: A. One set of three test cylinders shall be taken each day; one cylinder shall be tested at seven days and two cylinders shall be tested at 28 days. Copies of the test results shall be furnished to the Engineer. B. The concrete for the test cylinder shall be taken from the injection hose after the fine aggregate concrete has passed through the pump. Test cylinder to consist of nylon fabric in the form of a test sock to give an accurate in -place test result. PART 2 - PRODUCTS 2.01 FINE AGGRLGATE CONCRETE: Page 2 A. The fine aggregate concrete shall consist of a mixture of Portland cement, fine aggregate and water so proportioned and mixed as to provide a pumpable slurrc with a 28-day compressive strength of 2,000 psi when tested in accordance with ASTM C31 and C39. B. Transit mixed fine aggregate concrete shall be in accordance with ASTM C94. C. Portland cement shall conform to Federal Specification SS-C-192 or ASTM C150, Type I or Type II. D. Aggregate shall meet the requirements of ASTM C3333, except as to grading. Aggregate grading shall be well graded from the maximum size which can be conveniently handled with available pumping equipment. E. Water shall be clean, potable and free from injurious amounts of foreign matter. F. Admixtures, if utilized, shall contribute to the nature of the specifications. Pozzolith, or an equal water reducer conforming to ASTM C494, i may be used to reduce segregation, increase workabilityand improve strength and improve watertightness. If an air entrainmentagent is used, it shall improve resistance to freezing and thawing and shall reduce both bleeding and permeability. Other admixtures shall not be used. 2.02 FABRIC FORMWORK: A. Fabric forming material shall consist of multiple -panels, double layer, open -selvage fabric. The two fabric layers shall be no lighter than 20/15 count per inch, 1,000 denier. The fabric shall consist of yarn of nylon and polyesters. B. Fabric formwork shall be of the style and section type as specified in shop drawings and submittals. PART 3 - EXECUTION 3.01 SITE PREPARATION: A. Surfaces to be lined shall be trimmed and finished to the finished cross-section indicated on the plans. B. All stones, roots and waste material exposed on slopes shall be removed. C. Where necessary to bring a subgrade up to specified grade, suitable fill materials approved by the Engineer shall be utilized. Page 3 3.02 EQUIPMENT: A. Site preparation shall be performed with equipment suitable for the Project and project schedule. B. Site mixing of _`ine aggregate concrete shall be in full compliance with all ASTM standards. C. The pumping equipment shall be capable of the following: 1. Pumping a distance of 400 feet. 2. A variable flow rate. 3. Delivering 15 CY per hour minimum. 3.03 FABRIC PLACEMENT: A. The fabric installed shall be loosely positioned along the bank elvage lower layers. ppin panels. Each panel shall consist of two or more mill widths of open s construction formed by sewing upper layers to uer layers and lower layers to B. The panels shall be continuous and monolithic units for their full width, including the anchor trench portion. Each panel shall be measured approximately 3% more in each direction than the mat after injection to allow for proper filling out of the cross-section. Vertical laps may be necessary in some field situations. C. Adjacent fabric panels shall be joined, before fine aggregate concrete injection, by field sewing the two layers of fabric separately edge -to -edge. Where adjacent panels cannot be joined in this manner, or where called for on the drawings, adjacent panels shall be lapped a minimum of two feet. D. Dn a curved shoreline, or where there is a sharp or pronounced change in direction, excess material may be sewn off and tucked so as to provide continuity of mat. 3.04 CONCRETE INJECTION: A. Small cuts shall be made in the fabric to allow for the insertion of the injection hose or nozzle. The sequence of injecting the panels shall insure that no cold joint exists in any one panel and that the panels are filled to an adequate cross-section. Pre -sawn grout pockets can be used in underwater situations to reduce grout loss. B. Fine aggregate concrete shall be injected in such a way that.ezcessive pressure on the fabric is avoided.. C. Holes in the fabric left by the removal of the injection hose shall be temporarily closed by inserting a piece of burlap or similar material. The burlap shall be removed when the concrete is no longer fluid and the surface is firm to hand pressure. Foot pressure on the filled mat shall be restricted to an absolute minimum of one hour after pumping. D. Upon completion of the concreting operation, all the anchor trenches shall be backfilled with suitable material.
