Skvarla, III Secretary The Wilmington Regional Office received a complete Stormwater Management Permit Renewal Application and plan revision request for Markraft Cabinets on March 13, 2013. The Division is hereby notifying you that permit SW8 900811 has been renewed on March 15, 2013, and shall be effective until November 12, 2017. The plans previously approved on November 12, 1997, in accordance with the regulations set forth in Title 15A NCAC 2H.1000 effective September 1, 1995, remain in full force and effect. Please attach this cover letter and permit addendum to your High Density Permit originally issued on November 12, 1997. The revisions included and approved by this plan revision include the expansion of the parking lot over the loading dock area. The proposed improvements will reduce the impervious area by 130 square feet, which may be used for future development within the drainage area. Please add the attached plans to the previously approved plan set. The renewal and reissuance of this stormwater permit does not imply that the site is currently in compliance with the terms and conditions of this state stormwater permit. This permit is subject to the conditions and limitations as specified therein. Please pay special attention to the conditions listed in this permit regarding the Operation and Maintenance of the BMP(s), recordation of deed restrictions, procedures for changes of ownership, transferring the permit, and renewing the permit. Failure to establish an adequate system for operation and maintenance of the stormwater management system, to record deed restrictions, to transfer the permit, or to renew the permit, will result in future compliance problems. . If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing by filing a written petition with the Office of Administrative Hearings (OAH). The written petition must conform to Chapter 150E of the North Carolina General Statutes, and must be filed with the OAH within thirty (30) days of receipt of this permit. The permittee shall submit a permit renewal application request at least 180 days prior to the expiration date of this permit. The renewal request must include the applicable documentation and the processing fee. 4. Fountain are not allowed in the wet detention ponds until the volume of the permanent pool is demonstrated to be greater than 30,000 cubic feet. 5. If the use of permeable pavement is desired, this permit must be modified to add the permeable pavement conditions. J N Development, as the Owner of this Project, agrees to accept the responsibility for the inspection and maintenance of the storm water system, which affects said Project, pursuant to 15 NCAC 2H .1003 (g)-2. The Owner specifically agrees to carry out said responsibility with the following requirements: Specific Requirements: 1. Monthly Inspections: (a) The Owner shall provide the services of a person competent to inspect the storm water system on a monthly basis for catch basin or pipe blockages and detention pond operation. (b) This inspector shall remove any trash or debris which has collected in the catch basin's, pipes or detention pond, and report any more serious defects to the Owner. 2. Semi-annual Inspections: (a) At least once every six months the Owner shall have an inspection made to determine if excessive sedimentation or plant growth has occurred. (b) If sedimentation has occurred to the point that the general elevation of the bottom of the pond is more than one foot above the original pond bottom elevation, the sedimentation shall be removed at least to the original bottom elevation. (c) If the plant growth has progressed to the point that it interferes with the effectiveness of the pond, it shall be removed. Monthly, or after every runoff producing rainfall event, whichever comes first: A Inspect trash rack; remove accumulated debris, repair/replace if it is not functioning. B. Inspect and clear the orifice of any obstructions. If a pump is used as the drawdown mechanism, pimp operation will be checked. A log of test runs of the pump will be kept on site and made available to DEM personnel upon request. C. Inspect the pond side slopes and grassed inlet swales; remove trash, and repair eroded areas before the next rainfall event. D. If the pond is operated with a vegetated filter, the filter will be checked for sediment accumulation, trash accumulation, erosion and proper operation of the flow spreader mechanism. Repairs/cleaning will be done as necessary. II. Quarterly: A. Inspect the collection system (i.e. catch basins, piping, grassed swales) for proper functioning. Accumulated trash will be cleared from basin grates, basin bottoms, and piping will be checked for obstructions and cleared as required. B. Pond inlet pipes will be checked for undercutting, riprap or other energy dissipation structures will be replaced, and broken pipes will be repaired. III. Semi-annually: A. Accumulated sediment from the bottom of the outlet structure will be removed. B. The pond depth will be checked at various points. If depth is reduced 75% of original design depth or 3 feet whichever is greater, sediment will be removed to at least the original design depth. C. Grassed swales, including the vegetated filter if applicable, will be reseeded twice a year as necessary. POND MAINTENANCE REQUIREMENTS PAGE 2 IV. General: A. Mowing of the side slopes will be accomplished according to the season. Maximum grass height will be 6". B. Cattails are encouraged along the pond perimeter, however they will be removed when they cover more than '/2 the surface area of the pond. C. The orifice/pump is designed to draw down the pond in 2-5 days. If drawdown is not accomplished in that time, the system will be checked for clogging. The source of the clogging will be found and eliminated. D. All components of the detention pond system will be kept in good working order. Repair or replacement components will meet the original design specifications as per the approved sormwater plan. V. Special Requirement: I, Joe Jacobus hereby acknowledge that I am the financially responsible party for maintenance of this detention pond. I will perform the maintenance as outlined above, as part of the Certification of Compliance with Stormwater Regulations received for this project.
