Please describe all problems in detail: (Use additional sheets if needed) SCM 1: The wetland basin is oneratine correctly with good water level, deep pools, shallow zones, etc.. There are a lot of cattails and willows, which should be removed. There are also lots of wetland plants, which is - good. There is a significant amount of vegetation rp own up on the back side of the pond that blocks access around the SCM and needs to be mowed/removed. The outfall is in good working order. 9. BMP Condition: (check one) FAILED INSPECTION: BMP has MAJOR deficiencies (system failure) and must be repaired in order to function properly and operate as designed. BMP must be re -inspected following repairs. X CONDITIONAL APPROVAL: BMP has MINOR deficiencies (system operates, but repair is needed to ensure system does not fail), but can be certified conditionally. PASSED INSPECTION: BMP has no deficiencies and is hereby certified as functioning properly and as designed. 10. Queen Street Kinston, NC 28502 Subject: NOTICE OF INSPECTION Permit No. SW7090403 Modification Grantsboro Bojangles & Outparcels Pamlico County Dear Mr. McRae: On June 12, 2012, Mr. Samir Dumper of the Washington Regional Office of the Division of Water Quality (DWQ) inspected the Bojangles development in Grantsboro, Pamlico County to determine compliance with Stormwater Management Permit Number SW7090403 Modification, which was issued on April 1, 2010. DWQ site inspection revealed that the oroiect is in substantial compliance with the approved permit, however file review revealed that, as of today, the Engineer's Certification was not received. (see attached Compliance Inspection Report) Please be reminded that Section II (Schedule of Compliance) of the Stormwater Permit SW7090403 Modification, item 8, states the following: "Upon completion of construction and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications ......... Please submit the required information as requested above by July 16, 2012. If additional time is needed to complete these items, please submit a written request giving reasons by this date. Please be advised that you are required to comply with the terms, conditions and limitations of your Stormwater Management Permit under Title 15A North Carolina Administrative Code 2H .1003 and North Carolina General Statute 143-214.7, including operation and maintenance of your permitted stormwater system.
