Dear Mr. Horvath:
On May 29, 2014, 1 inspected the South Park Athletic Complex development in Elizabeth City to
determine compliance with Stormwater Management Permit Number SW7090224, which was issued on
March 27, 2009. The DWQ site inspection revealed that except for the issue listed below, the Project is in
substantial compliance with the approved permit. Please see attached Inspection report also.
• Designer's Certification not received. Per Schedule of Compliance Section of the stormwater
permit (Section III. 11), a certification must be received from the appropriate designer for the
system installed certifying that the permitted facility has been installed in accordance with this
permit.....
This Office requests that you respond to this letter in writing within 30 calendar days of receipt of this
Notice. Your response should be sent to this office at the letterhead address. Your response should
provide a written "Plan of Action" which outlines the actions you will take to correct the above issue and a
time frame for completion of those actions.
Thank you for your attention to this matter.
Please be advised that you are required to comply with the terms, conditions and limitations of your
Stormwater Management Permit under Title 15A North Carolina Administrative Code 2H .1003 and North
Carolina General Statute 143-214.7, including operation and maintenance of your permitted stormwater
system.
