Good 8 = Good 7 = Fair 5 -0 = Immediate Corrective Action Needed I A/P N Comments: A permit consultation is needed to discuss conditions at County Line Creek. There has been minor offsite into creek. Jerry Parker, DEO, has been notified and will review on 12- 10 -13. Please notify this office of any corrective actions from DWQ on sediment removal. Remarks and Recommendations: Project is out of complianace due to failure to provide adequate groundcover and failure to maintain erosion control devices. This project will be reviewed again within 5 working days. Please note the following corrective actions. 1) Remove sediment from stream as directed by DWQ. 2) Maintain sscf under new bridge at sta 19 +44. 3) Permanent s/m fill slopes from sta 16 +00 to sta 23 +001t of L. 4) Permanent s/m bare areas under new bridge. 5) Temp. s/m fill slopes from sta 17 +50 to sta 20 +00rt of L. 6) Temp. s/m exposures at borrow /waste site. 7) Sta 18 +2511 of L- Need to extend slope drain pipe into skimmer basin and not scour hole. 8) Sta 18 +6011 of L- Extend outlet end of slope drain to bottom of basin into a class -b rock pad. 9) Sta 21 +001t of L- Need to clean up area in front of slope drain inlet to ensure positive flow. NPDES was reviewed. Note that several of the above issues were noted on last months review dated 11- 20 -13, including maintanece needs on failed device above creek. There is no mention of any of these problems on NPDES reports. Good 8 = Good 7 = Fair 5 -0 = Immediate Corrective Action Needed Comments: Remarks and Recommendations: The "ICA" status on this project has been lifted at this time. A follow -up review was conducted with CEI, James Crissman, that indicated corrective measures have been addessed to bring project back into compliance. Contractor was not able to final grade fill slopes along It. side of project so exposures have been temp. seeded, including rt. side and bare areas under bridge. Encourage contractor to final grade and permanent s/m fill slopes as conditions allow. Failed device, sscf, above creek has been maintained and an additional line of defense, silt fence with sscf outlet, was installed just above this measure. Contractor has also replaced many of the wattles and excelsior matting on type -a checks in ditchlines that has weathered over the last several months. Slope drain inlets have been addressed for positive flow and slope drain outlets have been extended into basins with rock pads installed at outlet ends of pipe. The contractor is reminded to provide ground cover sufficient to restrain erosion as per NCG01 requirements. NPDES records were reviewed. The contractor has addressed the issues noted at time of last review. Contractor needs to continue with weeky reviews and within 24 hrs. after a 1/2 inch rainfall.
