Hi Hannah and Brad,
The subject project was a courtesy notification project that was started in June 2021 at the
intersection of US 17 and Douglass Rd in Onslow County and is now completed. I have visited the
project but we didn’t add it to our ride through schedule. The jurisdictional impacts were at the
outlet end of an existing pipe (stream) where 2 end sections of the existing 54” pipe and outlet
headwall had become separated. Please see the attached photos of the reset pipe and rip rap which
was permitted to fill the blowhole at the end of the pipe. This project also included signalization of
the intersection, island removal and replacement, and pavement marking adjustments.

Please see attached the photos of the outlet of the pipe. Please let me know if you have any
questions or comments or if you need any additional information. If the project is considered
complete we can return the completion forms through the electronic submittal or through email –
depending on which way is preferred.
