For your review we have enclosed five copies of the PCN Application Form, Attachment A — Additional Information, USGS Quadrangle Map, site plan, and the $240.00 application fee. The project titled "Wastewater Treatment Plant Equalization Tank" consists of installing approximately 800LF of force main piping and gravity sewer that crosses perpendicular to and continues parallel to an Unnamed Tributary to Foundry Branch. The construction described in this permit application will result in approximately 1,945 square feet of Zone 1 and approximately 4,602 square feet of Zone 2 buffer impacts within a maintained utility easement and 25 linear feet of stream disturbance. Please contact me with any questions regarding this permit application. Sincerely, Dewberry Engineers Inc. Marco Menendez, P.E.
