Project Description
3a.
Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
The project includes a 185 -LF reach of Haw Branch and a 500 -LF reach of UT1 to Haw Branch, both of which were
dredged as part of an effort to reduce flooding on the adjacent agricultural fields. Haw Branch included an exisitng 20 -LF
culvert at the road crossing that was replaced as a temporary impact. UT2 to Haw Branch included an existing 30 -LF pipe
at the road crossing that was replaced as as temporary impact. Additionally, a 160 -LF reach of a new culvert was
installed south and adjacent to the existing 30 -LF pipe on UT2 with a similar goal of reducing flooding. A 20 -LF reach of
this new 160 -LF culvert on UT2 will be removed to minimize permitting requirements. Once this 20 -LF section of pipe is
removed, the section will be restored to pre- existing conditions. An existing 20 -LF culvert on Haw Branch was replaced
recently and installed using pre -cast concrete blocks as the headwalls; the zone between the pipe and the headwalls
appears to have voids, indicating improper backfilling. The open channels are unstable, and dredge spoils piled on the
banks have increased bank heights such that stream to floodplain connections have been impacted. The primary land
use at the site is agriculture, but two single family houses are present near the western end of the parcel.

Explain the purpose of the proposed project:
The purposes of the project are to stabilize eroding stream banks on Haw Branch and UT1, remove roughly 20 -LF of the
newly installed 160 -LF culvert on UT2 in order to simplify the permitting process, restore the 20 -LF section of UT2 that will
include the culvert removal, to pre- existing conditions, and stabilize the backfill for the culvert on Haw Branch. The new
160 -LF (changed to 140 -LF) of culvert on UT2 is needed to assist in minimizing flooding on the adjacent agricultural fields
that are used for farming.
3e.
Describe the overall project in detail, including the type of equipment to be used:
The project will involve removing dredge spoils from stream banks, sloping stream banks and creating bankfull benches,
sowing seed on disturbed areas, and installing coir matting and live stakes on stream banks. A 20 -LF culvert on Haw
Branch was replaced. A 30 -LF culvert on UT2 was also replaced. A 20 -LF section of the new 160 -LF culvert on the UT2
to Haw Branch will be removed and the section will be restored to pre- existing conditions. At the Haw Branch culvert,
gaps between the culvert and headwalls will be chinked with suitably sized stone. The attached plans describe the
proposed work in more detail. Conventional construction equipment will be used, including a track mounted excavator,
dump truck and assorted hand tools. A pump- around flow diversion will be required for work on UT1 where more
significant in- stream grading will be required.

The project represents a long -term benefit to the streams by stabilizing eroding banks and reducing fine sediment inputs.
Stream impacts will be temporary and minimal. Construction phase best management practices will help minimize the short -
term impacts, as described in below.

The will begin only after all materials are stockpiled nearby. Equipment will work from the top of the stream banks and access
to the stream bottoms will be limited to designated areas of disturbance. A pump- around flow diversion will be used for work
on UT1. The contractor will be required to use only well maintained and functioning equipment and perform daily checks for
leaks. An emergency spill plan will also be required.
