On behalf of LLFlex, LLC (LLFlex), Hart & Hickman, PC (H&H) is providing a notification of benchmark exceedances of
Chemical Oxygen Demand (COD) in two stormwater samples collected from outfalls SDO-001 and SDO-002 during the
2021 3rd quarter monitoring event at the former Oracle Flexible Packaging, Inc. site in Winston-Salem, NC (Certificate of
Coverage: NCG030694). COD was detected above the benchmark of 120 mg/L for SDO-001 (139 mg/L) and SDO-002
(218 mg/L). To meet the Tier One response notification requirement, we are providing this notification. The DMR was
uploaded to the online portal and a signed hard copy will be mailed to the Winston-Salem Regional Office. A Tier One
response inspection will take place in this week to identify potential sources of COD and potential off -site, background
sources that may be flowing on to the site during storm events. Documentation of this inspection and actions taken as a

result of the inspection will be documented with the Stormwater Pollution Prevention Plan (SWPPP). Please let us know
