The Town of Gibsonville National Pollution Discharge Elimination System Phase II (NPDES Phase II) Stormwater Permit
(NCS000405) requires that structural SCMs be inspected annually to preserve and continue their function in controlling
stormwater runoff quality and quantity at the degree or amount of function for which the structural SCM was designed.

Inspection Report attachments as needed. Please attach digital photographs of the site and structural SCMs as
applicable.

Note: Applicable SCM Inspection Reports and confirmatory digital photographs summarizing required repairs must
be submitted to the Town following completion of the preliminary inspection. A re -inspection and certification must
be completed within 30 days of the failed preliminary report. It is strongly encouraged that the inspector be part of the
and maintenance process in a QA/QC role in order to ensure that repairs are being performed properly.
