Over the review period the facility was issued Ten Notice of Violations and Three Enforcements. Eight Notice of Violations and Three Civil Penalty assessments were due from late or failure to report eDMR's. The facility was reminded that DMR's are required to be postmarked no later than the last calendar day of the month following the completed reporting period. Ryan Swain is the ORC with no backup ORC. The Town of Creswell needs to designate a Backup ORC. Condition A. (2) of the NPDES permit required a staff gauge to be installed by September 30, 2018. On the day of inspection the staff gauge had not been installed. A staff gauge shall be installed no later than March 1, 2022 so weekly freeboard levels can be recorded. There is a well located beside the influent pump station. Special Condition A (2) in the NPDES permit required the well to be abandoned by October 31, 2018. On the day of inspection the well had not been abandoned. The well needs to be abandoned no later than March 1, 2022. On the day of inspection there was not a thermometer in the samplers. A thermometer needs to be maintained in the sampler to verify that the refrigerator temperature is below 6 degrees C. The gauge shall have readily visible permanent markings at inch or tenth of foot increments. Freeboard in the flow equalization pond shall not be less than two feet at any time as related to the lowest elevation point around the pond. Weekly freeboard records shall be maintained at the facility for a period of no less than five years and shall be made available to the Division upon request.
