Dear Mr. Gardner:
In accordance with your application received December 17, 2001, we are forwarding herewith Permit No.
WQ0020830, dated January 24, 2002, to the Town of Faith for the construction and operation of the subject
wastewater collection system extension. This permit shall be effective from the date of issuance until rescinded
and shall be subject to the conditions and limitations as specified therein. This cover Ietter shall be•considered a
part of this permit and is therefore incorporated therein by reference.
Please pay particular attention to Permit Condition 3 which requires that the wastewater collection
facilities be properly operated and maintained in accordance with 15A NCAC 2H .0227 or any individual system-
wide collection system permit issued to the Permittee.
Permitting of this project does not constitute an acceptance of any part of the project that does not meet
15A NCAC 2H .0200; the Division of Water Quality's (Division) Gravity Sewer Minimum Design Criteria
adopted February 12, 1996 as applicable; and the Division's Minimum Design Criteria for the Fast -Track
Permitting of Pump Stations and Force Mains adopted June 1, 2000 as applicable, unless specifically mentioned
herein. Division approval is based on acceptance of the certification provided by a North Carolina -licensed
Professional Engineer in the application. It shall be the Permittee's responsibility to ensure that the as -constructed
project meets the appropriate design criteria and rules. Failure to comply may result in penalties in accordance
with North Carolina General Statute §143-215.6A through §143-215.6C, construction of additional or
replacement wastewater collection facilities, and/or referral of the North Carolina -licensed Professional Engineer
to the licensing board.
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted to the Town of Faith
for the construction and operation of approximately 61,478 linear feet of 8-inch gravity sewer and 8,003 linear
feet of 10-inch gravity sewer, a 100-gallon per minute pump station (No. 1) with duplex pumps, on -site audible
and visual high water alarms, autodialer, and an automatically -activated stand-by generator with automatic reset; a
45-gallon per minute pump station (No. 2) with duplex pumps, on -site audible and visual high water alarms,
autodialer, and a portable generator quick -connect receptacle; a 100-gallon per minute pump station (No. 3) with
duplex pumps, on -site audible and visual high water alarms, autodialer, and automatically -activated stand-by
generator with automatic reset; and a 55-gallon per minute pump station (No. 4) with duplex pumps_, on -site
audible and visual high water alarms, autodialer, and portable generator quick -connect receptacle; as well as

approximately 3,365 linear feet of 4-inch force main, approximately 1,700 linear feet of 3-inch force main, a
approximately 555 linear feet of 2-inch force main to serve the Town of Faith (residential, school, church, and
commercial sources) as part of the Wastewater Collection Facilities to serve Town of Faith project, and the
discharge of 156,400 gallons per day of collected domestic wastewater into the City of Salisbury's existing
sewerage system, pursuant to the application received December 17, 2001 and in conformity with
15A NCAC 2H .0200; the Division's Gravity Sewer Minimum Design Criteria adopted February 12, 1996 as
applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and Force
Mains adopted June 1, 2000 as applicable; and other supporting data subsequently filed and approved by the
Department of Environment and Natural Resources and considered a part of this permit.
The sewage and wastewater collected by this system shall be treated in the City of Salisbury Wastewater
Treatment Facility (Permit No. NC0023884) prior to being discharged into the receiving stream.
. This permit, shall become voidable unless the agreement between the Town of Faith and the City of
Salisbury for the collection and final treatment of wastewater is in full force and effect:
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to
request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request
must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and
filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714. Unless
such demands are made, this permit shall be final and binding.

WASTEWATER, COLLECTION SYSTEM EXTENSION PERMIT
This permit shall be effective from the date of issuance until rescinded and shall be subject .to the following
specified conditions and limitations:
1. This permit shall become voidable unless the wastewater, collection facilities are constructed in accordance
with the conditions of this permit, 15A NCAC 2H .0200; the Division of Water. Quality's (Division) Gravity
Sewer Minimum Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design
Criteria for the Fast -Track Permitting of Pump Stations and Force Mains adopted June 1, 2000 as applicable;
and other supporting materials unless specifically mentioned herein.
2. This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
The wastewater collection facilities shall be properly maintained and operated at all times. The Permittee
shall maintain compliance with an individual system -wide collection system permit for the operation and
maintenance of these facilities as required by 15A NCAC 2H .0227. If an individual permit is not required,
the following performance criteria shall be met as provided in 15A NCAC 2H .0227:
a. The sewer system shall be effectively maintained and operated at all times to prevent discharge to land
or surface waters, and any contravention of the groundwater standards in 15A NCAC 2L .0200 or the
surface water standards in 15A NCAC 2B .0200. .
b. A map of the sewer system shall be developed prior to . January 1, 2004 and shall be actively
maintained.
c. An operation and maintenance plan shall be developed and implemented.
d. Pump stations that are not connected to a telemetry system shall be inspected at Ieast three times per
week until July 1, 2000; thereafter, pump stations shall be inspected at least daily, as defined in 15A
NCAC 2B .0503(5), until July 1, 2001; and thereafter, pump stations shall be inspected every day.
Pump stations that are connected to a telemetry system shall be inspected atleast once per week.
e. High -priority sewer lines shall be inspected at least once per every six-month period of time.
f. A general observation of the entire sewer system shall be conducted at least once per year.
g. Inspection and maintenance records shall be maintained for a period of at least three years.
h. Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with
15A NCAC 2B .0506(a), and public notice shall be provided as required by North Carolina General
Statute § 143-215.1 C.
4. This permit shall not be transferable. In the event there is a desire for the wastewater collection facilities
to change ownership, or there is a name change of the Permtttee, a formal permit request shall be submitted
to the Division accompanied by documentation from the parties involved, and other supporting materials as
may be appropriate. The approval of this request shall be considered on its merits and may or may not be
approved.

5. Construction of the gravity sewers, pump stations, and force mains shall be scheduled so as not to interrupt
service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface
waters of the State.
6. Upon completion of construction and prior to operation of these permitted facilities, a certification, a copy of
the construction record drawings, as well as supporting design calculations for any pump stations permitted
as part of this project shall be received from a North Carolina -licensed Professional Engineer certifying that
the facilities have been installed in accordance with this permit; 15A NCAC 2H .0200; the Division's
Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design
Criteria for the Fast -Track Permitting of Pump Station and Force Main adopted June 1, 2000 as applicable;
and other supporting materials. If this project is to be completed in phases and partially certified, you shall
retain the responsibility to track further construction approved under the same permit, and shall provide a
final certificate of completion once the entire project has been completed. A copy of the construction record
drawings, indicating the facilities constructed in the phase being certified, shall be submitted with each
partial certification. Mail the Engineer's Certification, one copy of the "Construction Record Drawings",
and one copy of the supporting design calculations to the Non -Discharge Permitting Unit, 1617 Mail Service
Center, Raleigh, NC 2769.9-1617.
7. A copy of the construction record drawings shall be maintained on file by the Permittee for the life of the
wastewater collection facilities.
8. FaiIure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H .0200; the
Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum
Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted June 1, 2000 as
applicable; and other supporting materials may subject the Permittee to an enforcement action by the
Division, in accordance with North Carolina General Statutes § 143-215.6A through §.143-215.6C.
9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement facilities.
10. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes, rules,
regulations, or ordinances that may be imposed by other government agencies (local, state and federal) which
have jurisdiction, including but not limited to applicable river buffer rules in 15A NCAC 2B .0200, erosion
and sedimentation control requirements in. 15A NCAC Ch. 4 and under the Division's General Permit
NCG0100000, and any requirements pertaining to wetlands under 15A NCAC 2B .0200 and 15A NCAC 2H
.0500.
11. Noncompliance Notification:
The Permittee shall report by telephone to the Mooresville Regional Office, telephone number
(704) 663-1699, as soon as possible, but in no case more than 24 hours or on the next working day, following
the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable of
adequate wastewater transport, such as mechanical or electrical failures of pumps, line blockage or
breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving waters without
treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in Ietter form within five days
following first knowledge of the occurrence. This report must outline the actions taken or proposed to be
taken to ensure that the problem does not recur.

Sonja,
You can just cut and paste the language from the three summaries beginning with, "A variance..." No need to include the background
justification stuff in the permit. I would highlight the requirement to fix the well situation in the permit in bold.
I'm going to log Tate's certification in as partiatuntil the well thing is corrected by whatever method the Town decides to meet the requirement
and we receive word of it. I'll note these details in BIMS. This will allow the Town to move forward with allowing most of
the residents/businesses to connect with exception of the 9 residents upstream of the well site until correction.

Gentlemen:
As you were copied on this request, The Town of Faith, on behalf of their engineer, Tate Lanning, PE, has requested a few variances on their
new collection system. Our responses to these variances are contained in the attached file memo. Of particular interest may be that we denied a
request to leave approximately 65' of SDR 35 sewer pipe within 100 feet of a public water supply well, at the recommendation of pubic water
supply staff. Mr. Lanning has been notified of that decision and has contacted the Town with regard to a solution.
The Town, once the permit modification is issued with the other variance approvals, line size changes, allocated flow and pump capacity
changes, can operate the system with exception of the 9 residences upstream of the well site until that has been corrected and the Division is
notified. Our regional office will be issuing this modification very shortly.
I'm not sure what your involvement is as of this point, but I thought you may want to know they have one more correction to make.

information, I'd appreciate it being passed on.

Variance to Gravity MDC
The Town of Faith was a previously unsewered community and recently had a new collection system
installed. Upon compiling the final documents for certification and operation, the Town engineer
discovered items not meeting our Gravity Sewer Minimum Design Criteria. While these are outlined
in the supporting documentation in more detail, they are summarized as follows: 1) SDR 35 PVC
pipe was installed 94 feet from a public water supply well; 2) a public 4-inch main was installed to
serve one single house across non -owned property; and 3) small portions of 10-inch and 8-inch
sewer were installed below minimum grade.
SUMMARY —
1) After contacting the regional engineer for public water supply (Mooresville Region) and discussing
the well situation, they do not agree with a variance. Current water supply regulations allow for
deviation from the 100-foot separation distance and from using ferrous pipe material (DWQ regs) if
water main materials and joints meeting water main standards are utilized. The pipe installed is SDR
35 PVC and was not tested to water main standards. A low pressure air test was used per ASTM
standards and it is unclear if there was any minor leakage noted. A variance to allow a PVC SDR 35
pipe within 100 feet of a public water supply well is denied. The Town shall either replace the line in
accordance with the regulations, reroute the line outside of 100-feet or abandon the well (with public
water supply concurrence). Certification that the situation has been corrected shall be submitted to
the Mooresville Regional Office, Surface Water Protection Section prior to allowing any connections
upstream of the line of concern (Lateral B-2 on the record drawings).
2) A public easement has been obtained and the 4-inch line can only serve one home. A variance is
granted to allow the 4-inch main along a permanent easement serving one residence to remain as
installed. No other connections shall be made to this line without prior approval of the Division. The
Town is responsible for this 4-inch line segment.
3) A segment of 10-inch pipe (318 feet) was installed at .26% when the minimum requirement is
.28%. A segment of of 8-inch pipe (134 feet) was installed at .37%. This deviation falls within our 10%
tolerance policy, will achieve a velocity of 2 fps at half full and surrounding segments are well above
the minimum. This flatter slope should not have much effect on the system operation. A variance is
granted to allow the line between MH 6 - 7(10') and E7 - E8 (8') to remain as installed.

DESCRIPTION OF REQUESTED MODIFICATIONS
The Project originally permitted under Permit No. WQ0020830 covered the construction
of a complete wastewater collection system to serve the Town of Faith in Rowan County.
Construction of the project commenced in December 2002 and was fully completed in
January 2005. The Town wishes to request modifications to the permit as originally issued.
The modifications requested are listed and described as follows:
1. Volume of Wastewater Flow:
A wastewater volume of 156,400 gals/day was permitted in the original application. A
total of 25 additional residential connections were installed during the construction process
— mainly to serve out-of-town residents whose property is adjacent to the new sewer lines,
and who had not signed up for sewer service prior to the issuance of the permit. The
volume attributable to the additional connections is estimated as follows:

The adjusted total wastewater volume to be permitted is 156,400 + 8,400 = 164,800 gpd.
Wastewater will be discharged into the City of Salisbury Collection System for eventual
treatment at the Salisbury -Rowan WWTF (NC0023884).
2. Pumping Stations — Pumping Rates
The Project included the construction of four (4) pumping stations with permitted
capacities as follows:
Pumping Station No. 1 (Non -clog) — Deal Street 100 gpm
Pumping Station No. 2 (Grinder) — Fraley Street 45 gpm
Pumping Station No. 4 (Non -clog) — Brown Street 100 gpm
Pumping Station No. 4 (Grinder) — Legion Club Rd 55 gpm
Modification — Station No. 2 - The grinder pumping station No. 2, as originally designed,
included the use of 2-inch PVC pipe, check valves and ball valves in the wet well without
external valve vaults. The Town of Faith arranged with the City of Salisbury to handle the
operation and maintenance of the pumping stations and the City requested that Pumping
Station No. 2 re -designed to utilize ductile iron pipe in lieu of PVC pipe in the wet wells
and to provide a separate valve vault to house the check valves and ball valves. The use of
ductile iron pipe in the wet well required that the size of the pump station piping be
increased from 2-inch to 3-inch, since 3-inch is the smallest size available for ductile iron
pipe. Station No. 2 had a short force main (<600 LF) and it was decided to increase the
entire force main to 3-inch. This necessitated an increase in the pumping rate to achieve
the desired velocity in the larger force main. The pumping rate was increased from 45

gpm to 87 gpm. This was accomplished utilizing the same 3.0 HP Myers pump as
originally specified.
Modification — Pumping Station No. 4 - Station No. 4 was also a grinder station. However,
clue to the higher head and capacity requirement, it was designed with 3-inch ductile iron
piping in the wet well. The 3-inch valves were located in a separate vault. Due to the
higher operating head requirement, the City of Salisbury requested that the Town of Faith
use ductile iron force main pipe in lieu of the PVC force main originally proposed. Since
the ductile iron pipe has a slightly larger inside diameter than the PVC pipe, the pumping
rate was increased from 55 gpm to 64 gpm to achieve the desired velocity. This was
accomplished utilizing the same 5.0 HP Myers pump as originally proposed.
In summary, the Town of Faith requests that the permit be modified to increase the
pumping rates at Pumping Station No. 2 and No. 4 as follows:
Pumping Station No. 2 increase to 87 gpm (was 45 gpm)
Pumping Station No. 4 increase to 64 gpm (was 55 gpm)
No modification is requested for the non -clog Pumping Stations No. 1 and No. 3.
3. Force Main Sizes
The final lengths of force mains 3-inch and smaller differ from the lengths shown in the
permit. The revised lengths are as follows:
3-inch diameter force main 2,255 LF (was 1,700 LF)
2-inch diameter force main 0 LF (was 555 LF)
1-1/2 inch diameter force main 95 LF (was 0 LF)
As previously noted in section 2 above, the force main at Pumping Station No. 2 was
increased from 2-inch to 3-inch at the request of the City of Salisbury and the 2-inch force
main was eliminated.
The 1'/z inch force main represents the length of force main extended beyond the frontage
of a residential lot that could not be served by gravity and was served instead by a small
grinder pump station located on the lot. The force main is located in public right-of-way
and will be maintained by the Town of Faith.

BACKUP FOR REQUESTED VARIANCES
Several variances are indicated on the Application for Permit Modification. The details for
the requested variances are presented below.
1. Separation from a Public Water Supply Well — Sewer Lateral B-2 is Non -compliant
The Gravity Sewer Minimum Design Criteria stipulate in Section VI, Item B, Paragraph 2
that a minimum separation of 100 feet shall be maintained between sewer lines and water
supply wells. If the minimum separation cannot be maintained, the sewer line is required
to be constructed of "ferrous sewer pipe with joints equivalent to public water supply
design standards. .. "
The public water supply well on Gardner Street in Faith was not located by survey during
the design phase of the project, and the proximity of this well to the sewer line on Gardner
Street (Lateral B-2) was not observed or noted until the final inspection for the wastewater
collection system. Following the final inspection, the Engineer surveyed the Location of the
well and noted the location on the as -built drawing for Lateral B-2. The survey indicates
that the center of the well is 94 feet from the center of the PVC sewer pipe (SDR-35)
between Manholes B49 and B19. Approximately 65 feet of the PVC' sewer pipe is within
the 100-foot restrictive radius.of the Construction
Drawings (Record Drawings).
There is approximately 1,000 feet of new sewer in Lateral B-2 upstream of the point of
concern. Following its construction, Sewer Lateral B-2 was observed to have no
measurable infiltration entering the system from above manhole B19. The sections of pipe
upstream of Manhole B49 were subjected to low-pressure air tests on September 11, 2003
in accordance with applicable ASTM Standards. The sewer line met or exceeded the test
requirements for minimum pressure and duration.
The Gardner Street well has served the Town of Faith for approximately 37 years and
currently supplies approximately 8 to 10 percent of the Town's current water demand.
Although it is the lowest producing well on the Faith Water System in terms of capacity,
the Town would like to keep it in service.
To correct the problem would require either replacing the affected sewer line with ductile
iron pipe or abandoning the well. To replace the affected sewer line with ductile iron pipe
would be expensive and undesirable .in view of the. fact that the street was recently
resurfaced following construction of the sewer system. Abandoning the well is expected to
be significantly less expensive, but would have some negative impact on the Town's water
supply.
2. Minimum Gradients for Gravity Sewer Pipe- Non -compliant at Two Locations
Section 1I1, item D, Paragraph 1 (d) of the Gravity Sewer Minimum Design Criteria,
establishes the minimum slopes for 10-inch and 8-inch gravity sewer pipe as noted below.

The final "As -Built" survey of the constructed sewers reveals that the required minimum
slopes were not met at two locations. These locations noted below with the "as -built"
slope calculations.

Elevation of invert out at Manhole No. 7 (as surveyed) 715.47
Elevation of invert in at Manhole No. 6 (as surveyed) 714.64
Total fall between manholes 0.83 feet
Distance between manholes (as surveyed) 318.60 feet
Slope between manholes = 0.83/318.60/100 = 0.26 feet/100 feet
This does not meet the minimum sloe requirement of 0.28 feet/100.

3. Minimum Size Requirement for Public Sewers — Non -compliant at One Location
Under DENR 0200 Regulations - 15A NCAC 02H .0219 (i) (2) F — the minimum size of
public gravity sewers is required to be 8-inch diameter.
A 4-inch sewer was constructed to serve a single residential lot. The property being served
by the 4-inch sewer is located out of Town and the owner of the lot had not requested the
service until after the sewer lines in the affected area had been installed. The 4-inch line in
question is shown on Sheet No. 16 of the Construction Record Drawings. The line was
installed in an easement across the neighbor's property to the rear of the property being

served. All surrounding properties are being served from other service connections and the
4-inch line in question cannot be used to serve other property. The Town will maintain the
4-inch line in question from the point of connection to the 8-inch collector main in the
adjacent street to the property being served. Therefore, it is considered a public sewer, but
does not meet the 8-inch minimum size requirement for public sewers. See=:t}e attached

Construction of the Wastewater Collection Facilities to serve the Town of Faith has now been
completed. In the process of preparing the record drawings we noted several items that will
require modification to the original permit. On behalf of the Town of Faith I am enclosing the
Town's application for a permit modification. The application also lists three items that do not
comply with the Gravity Sewer Minimum Design Criteria and/or with the applicable regulations of
15A NCAC 2H .0200. Variances are requested for these items.
The enclosed permit application package consists of the items listed below with comments:

contains the Applicant's Certification signed by W. Gary Gardner, Mayor of Faith. The

letter accepts the modified flow volume of 164,800 gpd. The volume of wastewater accepted
in the original permit was 156,400 gpd. This letter was addressed to Ms. Marie Doklovic in
