Dear Mr. Trask:
Division of Water Resources’ Central and Regional staff has reviewed the application package received September 13, 2021. However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on December 22, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through Q, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. 1. Within Item IV.10 there are three additional piezometers proposed that were not included in the existing permit’s Attachment C. A search of our database found that proposed coordinates were not available for any of the piezometers (PZ-1 through PZ-10). Please provide the proposed latitude and longitude coordinates to the sixth decimal degree and update the table accordingly.
2. The proposed modification includes additional treatment units/equipment; however, Item V.11. was not completed. Please provide the facility design criteria for the proposed modifications including manufacturer or material and the plan sheet and specification references, where appropriate.
D. Property Ownership Documentation:
1. The proposed groundwater lowering system appears to traverse and ultimately outfall on an adjacent parcel (Parcel No. 3245-80-3934-0000). This adjacent parcel is owned by Springfield
Plantation II Inc. per the Pender County Register of Deeds and is not owned by the Permittee (HCT Pender, LLC). Please provide property ownership documentation that meets the requirements of 15A NCAC 02T .0704(f) for all properties associated with the wastewater treatment and high-rate
infiltration facilities.

I. Engineering Plans:
1. Sheet C8 indicates that the proposed carbon absorber is to be located on the same concrete access
pad as the solids dumpster and sludge handling hookup. Are bollards proposed to provide a barrier between the absorber and vehicular traffic? Additionally, is enough horizontal clearance provided between the sludge wasting quick connect and the absorber to allow for hook up? Please provide additional information and revise the drawings if needed.
2. Detail drawings and/or profile view for the covers were not provided. The provided plan views were limited and did not indicate proposed material or if these features are to be sloped or domed. Please provide additional information regarding the equalization and digester tank covers.
J. Specifications:
1. Specifications for the proposed modifications (the tank covers and air scrubber) were not provided with the submittal. Pursuant to 15A NCAC 02T .0704(c)(2), specifications are to be provided describing materials to be used and means for ensuring quality and integrity of the finished product.
This document is to be signed and sealed by a Professional Engineer licensed in North Carolina. Please provide a copy of the signed and sealed specifications for the proposed facility modifications.
K. Engineering Calculations:
1. Engineering calculations for the proposed treatment unit (air scrubber) were not provided. Pursuant to 15A NCAC 02T .0704(c)(3), engineering calculations are to be provided for each treatment unit.
This document is to be signed and sealed by a Professional Engineer licensed in North Carolina. Please provide a copy of the signed and sealed engineering calculations for the proposed treatment unit.

1. An updated Operation & Maintenance (O&M) plan was not provided. The proposed modifications include additional treatment units and components that will require additional operation and maintenance considerations. Please provide an updated O&M plan meeting the requirements of 15A NCAC 02T .0707.

1. Condition I.6 of the existing permit indicates that two existing wells were to be permanently abandoned with the appropriate Well Abandonment Record (Form GW-30) submitted to the Water Quality Permitting Section. Has this compliance schedule been completed?

Q. Recommendations: (Response not required)
a. The permitted high-rate infiltration system has Basin #1 located at the north end of the site near the
previously permitted treatment facility location and Basin #3 located at the south end of the site near the proposed treatment facility location. Within the existing permit, previously approved plans, and the engineering plans associated with this submittal, Basin #1 is associated with Phase 1 of construction. Similarly, Basin #2 is associated with Phase 3 and Basin #4 associated with Phase 4. Please be aware that no modifications to the high-rate infiltration disposal system or the associated groundwater control and disposal system were proposed, and Basin #1 will continue to be associated with Phase 1 despite the proximity of Basin #3.
