Dear Dr. Doby:
In accordance with your application received on October 25, 2004, and additional information
received on November 10, 2004, we are forwarding herewith Permit No. WQ0005740, dated November
16, 2004, to the Rowan -Salisbury Board of Education for the construction and operation of the subject
wastewater collection system extension. This permit shall be effective from the date of issuance until
rescinded, shall void Permit No. WQ0005740 issued November 12, 1991, and shall be subject to the
conditions and [imitations as specified therein. This cover letter shall be considered a part of this permit
and is therefore incorporated therein by reference. The modification includes the addition of
approximately 822 linear feet of 8-inch gravity sewer, an 80-gallon per minute pump station,
approximately 1,372 linear feet of 4-inch force main, approximately 200 linear feet of 5-inch'force main,
the addition of 5,500 gallons per day of wastewater flow; and the abandonment of the existing 80-gallon
per minute lift station structure, also the abandonment of 1,513 linear feet of 8-inch gravity sewer and
approximately 1,410 linear feet of 4-inch force main.
Please pay particular attention to Permit Condition 3 which requires that the wastewater
collection facilities be properly operated and maintained in accordance with 15A NCAC 2H .0227 or any
individual system -wide collection system permit issued to the Permittee.
Permitting of this project does not constitute an acceptance of any part of the project that does
not meet 1) 15A NCAC 2H .0200; 2) the Division of Water Quality's (Division) Gravity Sewer Minimum
Design Criteria gdopted February 12, 1996, as applicable; 3) and the Division's Minimum Design
Criteria for the Fast -Track Permitting of Pump Stations and Force Mains adopted June 1, 2000, as
applicable, unless specifically mentioned herein. Division approval is based on acceptance of the
certification provided by a North Carolina -licensed Professional Engineer in the application. It shall be
the Permittee's responsibility to ensure that the as -constructed project meets the appropriate design
criteria and rules. Failure to comply may result in penalties in accordance with North Carolina General
Statute §143-215.6A through §143-215.6C, construction of additional or replacement wastewater

8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2H .0200; the
Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's
Minimum Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted
June 1, 2000 as applicable; and other supporting materials may subject the Permittee to an
enforcement action by the Division, in accordance with North Carolina General Statutes §143-
215.6A through §143-215.6C.
9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take immediate corrective action, including
those as may be required by this Division, such as the construction of additional or replacement
facilities.
10. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances that may be imposed by other government agencies (local, state
and federal) which have jurisdiction, including but not limited to applicable river buffer rules in 15A
NCAC 2B .0200, erosion and sedimentation control requirements in 15A NCAC Ch. 4 and under the
!Division's General Permit NCG010000, and any requirements pertaining to wetlands under 15A
NCAC 2B .0200 and 15A NCAC 2H .0500.
11: • Noncompliance Notification:
The Permittee shall verbally report to a Division of Water Quality employee at the Mooresville
Regional Office, telephone number [phone], as soon as possible, but in no case more than
24 hours or on the next working day, following the occurrence or first knowledge of the occurrence
of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable
of adequate wastewater transport, such as mechanical or electrical failures of pumps, line
blockage or breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving waters
without treatment of all or any portion of the influent to such station or facility.

Voice mail messages or faxed information is permissible, but shall not be considered as the initial
verbal report. Overflows and spills occurring outside normal business hours may also be reported
to the Division of Emergency Management at telephone number [phone] or (919) 733-
3300. Persons reporting any of the above occurrences shall file a spill report by completing Part I
of Form' CS-SSO (or the most current Division approved form), within_five, days following first
knowledge of the occurrence. This report shall outline the actions taken or proposed to ensure that

Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been
installed in accordance with this permit, the approved plans and specifications. Mail the
Certification to the Permits and Engineering Unit, P.O. Box 29535, Raleigh, NC 27626-0535.
8. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a
minimum of five years from the date of the completion of construction.
9. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee
to an enforcement action by the Division of Environmental Management in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.
20. The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be imposed by other government agencies
(local, state, and federal) which have jurisdiction.
11. The Permittee shall provide for the pump station and force main the following items:
a. Pump on/off elevations Iocated so that 2-8 pumping cycles may be achieved per hour in the
pump station.
b. An air relief valve located at all high points along the force main.
c. A screened vent for the wet well.
d. Fillets located in the wet well at the intersection of the flooring and sidewalls.
e. Three feet of cover (minimum) over the force main or the use of ferrous material where
three feet cannot be maintained.
f. Sufficient devices which will protect the pump station from vandals.
g. Flood protection if the pump station is located below the 100-year flood elevation.
12. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement wastewater collection
facilities.
13. Noncompliance Notification:
The Permittee shall report by telephone to the Mooresville Regional Office, telephone no.
704/663-1699 as soon as possible, but in no case more than 24 hours or on the next working day
following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that render the facility
incapable of adequate wastewater transport such as mechanical or electrical failures of
pumps, line blockage or breakage, etc.

14. This permit shall become voidable unless the agreement between Rowan -Salisbury Board of
Education and Rowan County for the collection and final treatment of wastewater is in full force
and effect.
15. No wastewater shall be made tributary to this collection system until such time as the Rowan
County Second Creek #1 Wastewater Treatment Facility is constructed and operational.

Pursuant to your request for flow acceptance of the potential domestic waste stream
of 16,500 gallons per day (maximum) from the West Rowan Middle School, Rowan
County has reviewed operational data and plant capacity of the receiving facility,
Rowan Second Creek WWTP (Permit No. NC0078361).. The following information is
provided for your use.
The receiving facility has a design capacity and permitted flow of .030 MGD and
currently serves the West Rowan Middle School and Meridian Automotive Systems,
Inc., a manufacturer of truck body parts. The current average Inflow demand on the
facility is less than half of the design capacity.
With concurrence of Salisbury -Rowan Utilities, the operator of thls facility, Rowan
County agrees to accept the requested flow amount, which represents an Increase of
5500 gallons per day over the previously allowed amount.

Rowan County and it maintained by Rowan -Salisbury 'Utilities,
Due to the widening project, the existing pump station will have to be relocated approximately 275
feei east of its current location and moved outside of die proposed right-of-way. New 8" PVC
gravity sewer line will be installed from the pump station, under the proposed US 70 widening, to at)
existing manhole, New 4" PVC and 5" 1,1DPE;:, (under creek) force main will be installed from the
proposed pump station, running parallel and just inside the fleNV right-of-way line, to the existing
WWFF. 'fhe force wain is being relocated clue to conflicts With the proposed bridge construction
over North Second Creek and various storm drainage pipes.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina
as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
ROWAN-SALISBURY BOARD OF EDUCATION

FOR THE
construction and operation of approximately 3,577 linear feet of 8-inch gravity sewer, a 80 GPM pump
station with duplex pumps and high water alarms, and approximately 1,392 linear feet of 4-inch force
main to serve Rowan Middle School and the discharge of 11,000 GPD of collected domestic wastewater
into the Rowan County's existing sewerage system, pursuant to the application received October 22, 1991
and in conformity with the project plan, specifications, and other supporting data subsequently filed and
approved by the Department of Environment, Health and Natural Resources and considered a part of this
permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with the
conditions of this permit, the approved plans and specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. The facilities must be properly maintained and operated at all times.
4. The sewage and wastewater collected by this system shall be treated in the Rowan County Second
Creek #1 Wastewater Treatment Facility prior to being discharged into the receiving stream.
5. This permit is not transferable. In the event there is a desire for the facilities to change ownership,
or there is a name change of the Permittee, a formal permit request must be submitted to the
Division of Environmental Management accompanied by an application fee, documentation from
the parties involved, and other supporting materials as may be appropriate. The approval of this
request will be considered on its merits and may or may not be approved_
b. Construction of the sewers, pump station(s) and force main shall be scheduled so as not to
interrupt service by the existing utilities nor result in an overflow or bypass discharge of
wastewater to the surface waters of the State.

7. Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been
installed in accordance with this permit, the approved plans and specifications. Mail the
Certification to the Permits and Engineering Unit, P.O. Box 29535, Raleigh, NC 27626-0535.
8. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a
minimum of five years from the date of the completion of construction.
9. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee
to an enforcement action by the Division of Environmental Management in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.
10. The issuance of this permit does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be imposed by other government agencies
(local, state, and federal) which have jurisdiction.
11. The Permittee shall provide for the pump station and force main the following items:
a. Pump on/off elevations located so that 2-8 pumping cycles may be achieved per hour in the
pump station.
b. An air relief valve Iocated at all high points along the force main.
c. A screened vent for the wet well.
d. Fillets located in the wet well at the intersection of the flooring and sidewalks.
e. Three feet of cover (minimum) over the force main or the use of ferrous material where
three feet cannot be maintained.
f. Sufficient devices which will protect the pump station from vandals.
g. Flood protection if the pump station is located below the 100-year flood elevation.
12. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement wastewater collection
facilities.
13. Noncompliance Notification:
The Permittee shall report by telephone to the Mooresville Regional Office, telephone no.
704/663-1699 as soon as possible, but in no case more than 24 hours or on the next working day
following the occurrence or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that render the facility
incapable of adequate wastewater transport such as mechanical or electrical. failures of
pumps, Iine blockage or breakage, etc_
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving
waters without treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form within
15 days following first knowledge of the occurrence. This report must outline the actions taken or
proposed to be taken to ensure that the problem does not recur_
14. This permit shall become voidable unless the agreement between Rowan -Salisbury Board of
Education and Rowan County for the collection and final treatment of wastewater is in full force
and effect.
15. No wastewater shall be made tributary to this collection system until such time as the Rowan
County Second Creek #1 Wastewater Treatment Facility is constructed and operational.

With respect to our visit to your office and subsequent telephone conversation regarding
the US 70 West Rowan Middle School Lift Station, please be advised that the Rowan
Salisbury School System is prepared to resume authority and responsibility for its
operation. In order to be fully compliant with Permit No. WQ0005740 issued Nov.I 6,
2004, we will need several items presented or confirmed, 'by your office. For your
convenience, please find attached a copy of documents held by RSSS and NC DENR
DWQ to support these requests.
As required by item 3 b., we will need a map of the sewer system.
In accordance with the letter to Dr. Doby dated Nov. 16, 2004, considered part of the
permit, page 2 paragraph one, please advise, confirm, or explain the telemetry as stated
"and a portable generator receptacle with telemetry". This may have a direct bearing on
item 3 d.
As required by item 6, we and Mooresville Regional Office DENR DWQ need a
completed, Engineering Certificate, fully executed along with supporting documents.
Please send Mooresville documents attention: Dee Browder.
As required by item 7, we will needa complete set of construction record drawings along
with any additions especially regarding the vacuum/siphon issue.
Ad.ditionally, we would appreciate any copies of correspondence regarding the trimming
of the impellers on the two pumps in the lift station. After meeting with Wayne Harrison,
the pump manufacturer representative, ‘ve were informed. and accept that the pumps are
no longer covered by the original warranty.
In closing, 1 would like to thank you for all your help and cooperation throughout the
construction and problem solving phases of this project, You must be relieved to be able
to mark one more thing off your 'to-dolist.
