On October 30, 2013, the Division of Water Resources (DWR) received a Year 6 Monitoring Report for
the Forrest Creek Buffer Mitigation Bank from McAdams Company, on behalf of EBX -EM, LLC. On
November 8, 2013, Katie Merritt, with DWR, requested additional information regarding the remediation
efforts taken by EBX -EM, LLC in response to the letter dated May 3, 2013 from DWR. On November
11, 2013, Ms. Merritt received a satisfactory response to the information request, and hereby approves the
Year 6 Monitoring Report.
Per the letter dated May 3, 2013 from DWR, the subject site has 2 more years of monitoring.
