Terra Consulting, LLC is submitting the enclosed revised Notice of Intent (NOI) for General Permit No.
NCG 140492 on behalf of C.W. Matthews Contracting Co., Inc. for the operation of a concrete batch plant
to be located at the 1-840/Ohenry Blvd. Interchange in the City of Greensboro in Guilford County. The
original NOI assigned the number NCG140492 was submitted by McCarthy Improvement Company in
December 2020. McCarthy Improvement Company has been acquired by C.W. Matthews Contracting
Co., Inc. of Marietta, Georgia. Therefore, the enclosed NOI and attachments include a name change as
well as a revised site plan.
The project is currently schedule to beginning paving in the March 2022.
No land disturbance permit is required for this site because the plant will be installed within the permitted
area of the NCDOT Guildford County Greensboro Outer Loop project.
The following items are enclosed:

a. All notices of intent to be covered under this General Permit shall be signed as follows:
(I) For a corporation: by a responsible corporate officer. For the purpose of this permit, a responsible corporate officer means: (a) a
president, secretary, treasurer or vice president of the corporation in charge of a principal business function, or any other person who
performs similar policy or decision making functions for the corporation, or (b) the manager of one or more manufacturing production or
operating facilities employing more than 250 persons or having gross annual sales or expenditures exceeding 25 million (in second quarter
1960 dollars), if authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures.
(2) For a partnership or sole proprietorship: by a general partner or the proprietor, respectively; or
(3) For a municipality, state, federal, or other public agency: by either a principal executive officer or ranking elected
official.
Certification: North Carolina General Statute 143-215.6 B(i) provides that:
Any person who knowingly makes any false statement, representation, or certification in any application, record, report, plan, or other
document filed or required to be maintained under this Article or a rule implementing this Article; or who knowingly makes a false statement of
a material fact in a rulemaking proceeding or contested case under this Article; or who falsifies, tampers with, or knowingly renders inaccurate
any recording or monitoring device or method required to be operated or maintained under this Article or rules of the [Environmental
Management) Commission implementing this Article shall be guilty of a Class 2 misdemeanor which may include a fine not to exceed ten
thousand dollars ($10,000).

3. Physical location information:
The facility is located at the construction site of the Guilford County Greensboro Outer Loop (I-840) on the
northeast side of Greensboro. The site is located on the right-of-way for 1-840.
Driving directions to the site from DEQ Regional Office in Winston-Salem, NC:
Get on US-52 S from W Hanes Mill Rd and follow US-52 S, US-421 S and 1-40 E to US-220 N/US-29 N/US-70
E/S Ohenry Blvd in Greensboro. Take exit 223 from I-40 E and drive north on Ohenry Blvd approximately 4.4
miles to 161 Street Exit. Take the 161 Street Exit and turn left on 1611 Street. Proceed approximately 0.5 mile to
Summit Avenue and turn right. Follow Summit Avenue north approximately 2.9 miles to Pineneedle Dr., then
turn right on Pineneedle Dr. Drive about 320 feet SSE on Pineneedle, then turn left onto Assembly Rd and
proceed approximately 0.4 mile. The site is located on the left (north) side of Assembly Rd, approximately 150
feet before the end of Assembly Rd at the cul-de-sac.
8. Description of Industrial Activities
This batch plant will provide concrete for the paving of the Guilford County Greensboro Loop from US 29, North
of Greensboro. The plant will be relocated to this location during the first quarter of 2021. However, the plant
will not begin operating until September 2021. No materials will be inventoried or stockpiled at this location until
shortly before the plant is anticipated to begin operations.
This facility will manufacture concrete, which is transported by dump truck to the area where lanes are being paved.
The primary components of the facility include the production plant, aboveground storage tanks that contain diesel
fuel and concrete admixtures, storage trailers and a silo that contain cement, as well as sand and aggregate stored
in open stockpiles. The manufacturing process requires moving aggregate and sand from stockpiles to the
processing plant using front-end loaders and conveyors. Cement is pneumatically added to the sand and aggregate
and processed in the mixing drum with water. The concrete is then loaded into dump trucks for delivery to the
paving project.
12. Type of Wastewater
A combination of wastewater and stormwater will be discharged from this facility's outfall intermittently during
the approximately 6 months the plant is expected to operate at this site. Wastewater from the truck bed and drum
washout will be comingled with stormwater only when the plant is in operation. On the days the plant is
operating, the wastewater described in Attachment B will be generated.
17. Wastewater Treatment Alternatives Review
(b) This facility will be operated at this site for approximately nine (9) months. Because of the cost involved
and potential delays to the NCDOT project as a result of permitting and construction required to temporarily
direct the wastewater from the identified sources to the sewer system, this option is not considered feasible for the
short period of time the plant will operate at this location.
(c) Surface or Subsurface Disposal Systems
(i) It is technically possible to use the recycled water from the truck bed washout recycling system for
dust suppression. It is also possible to treat and dispose of wastewater from the site through the existing
sediment basins installed for the highway construction.
(ii) It is possible to use the recycled water from the truck bed washout recycling system for dust
suppression as well as for re -use in the washout system.
(iii) These methods of surface disposal and re -use are feasible and will be implemented.
18. Best Management Practices (BMP)
This facility is located within the right-of-way for I-840 and many of the erosion and sediment control BMPs have
been installed by E.S. Wagner, the prime contractor on this project, according to NCDOT approved plans. BMP
at the site include:
(1) All disturbed areas within the plant site will be stabilized with 4 to 6 inches of graded aggregate base (GAB)
or vegetated with permanent vegetation.
(2) Runoff from the entire site flows to a Modified Silt Basin that discharges to a Skimmer Sediment Basin which
discharges to the outfall.
(3) Inlet protection at the Silt Basin and outlet protection from the Skimmer Sediment Basin.
(4) A Temporary Storm Drainage channel located along the northern site boundary which divers all runoff from
the eastern portion of the site, as well as run-on from the highway construction located to the north of the site,
to the silt basin.
(5) Check dams located in the Temporary Storm Drainage channel.
(6) Containment and diversionary beans directing all runoff to the silt basin and skimmer sediment basin.
(7) All parts and maintenance oil, grease, and fluids will be stored in closed containers inside the parts trailer.
Diesel fuel kept on site to fuel material handling equipment will be stored in a 1,000-gallon double -wall steel
tank inside of a portable containment dike.
(8) The double -wall steel diesel storage tank used to store fuel for the generator is located inside the generator
trailer.
(9) Regular inspections of potential sources of pollution are conducted.
24. Discharging wastewater to a BMP
This facility is located within the right-of-way for I-840 and many of the erosion and sediment control BMPs have
been installed by E.S. Wagner, the prime contractor on this project, according to NCDOT approved plans.
Runoff from the entire site flows to a Modified Silt Basin that discharges to a Skimmer Sediment Basin. The only
outfall, which is designated as Outfall No. 1, is located at the discharge pipe from the skimmer. Discharge from
the Skimmer Basin flows into a culvert installed under a newly constructed onramp to I-840 into a series of
unnamed tributaries and ultimately flows into North Buffalo Creek. The North Buffalo Creek segment is 16-11-
14-lb, described as from North Buffalo Creek WWTP to Buffalo Creek.

Description of Processes Producing Wastewater
Three processes at this facility will potentially contribute wastewater to the effluent from the outfall.
Wastewater Source 1:
Wastewater Source 1 is the concrete batch plant mixing drum. The drum on the plant will be washed out once per
day, on days the plant is operating. The wastewater produced from each washout is approximately 800 gallons
per day. This wastewater is captured in the upturned bucket of a front end loader and transported to the closest
truck bed washout recycling pit located on the site, where it is discharged from the bucket.
Wastewater Source 2:
Wastewater Source 2 is the truck bed washout used to washout the beds of the dump trucks used to transport the
concrete to the area being paved. The plant anticipates producing a maximum of 180 truckloads per day and the
trucks will be washed out after every second trip. Each washout will use approximately 50 gallons of water. The
anticipated volume of wash water generated will be approximately 4,500 gallons per day. The wash water will be
contained and recycled through a series of three settling pits designed to allow suspended particles to settle out of
the wash water. The dimensions of each pit will be 20 feet by 20 feet by 8 feet deep. The combined holding
capacity of the pits is approximately 200,000 gallons. The water in the third pit will be recycled as wash water or
used for dust suppression on the site. Solids will be removed from each pit when the volume occupied by the
solids equals one half of the designed holding capacity of the pit. The solids are allowed to dry and are properly
disposed of. If wash water is spilled or leaks from the recycling system or the washout area, it would flow to a silt
basin and skimmer sediment basin at the northwest corner of the site, discharge from the skimmer basin flows into
a culvert under the off -ramp under construction for I-840. The discharge flows through several unnamed
tributaries and ultimately discharges to North Buffalo Creek. Fifteen inches of freeboard will be maintained in
the third washout pit at all times to accommodate a 10 year rain event (4.9 inches) over the three ponds.
Wastewater Source 3:
Wastewater Source 3 includes the aggregate stockpiles. The aggregate stockpiles will be wetted using a lawn
sprinkler and hose. The sprinkler will be rotated around each stockpile during the day to cover all parts of each
pile. Wetting the aggregate stockpiles is done to produce evaporative cooling of the stone. The evaporation of
the water during this sprinkling process reduces the volume of water that ultimately flows away from the
stockpiles. Wetting is not used on the sand stockpile. Per DOT specifications, both the aggregate and sand are
washed prior to being delivered to the site, reducing the potential fines in stormwater runoff from the area covered
by the stockpiles. Wetting of the aggregate stockpile will be conducted up to 24 hours per day when the plant is
operating during summer months. The wastewater from the aggregate stockpile will consist of water from the
wetting of the stone and stormwater that falls on the pile. Wastewater from the sand stockpile will consist of only
stormwater that falls on the pile. Stormwater and wastewater from the stockpiles will flow to a silt basin and
skimmer sediment basin at the northwest corner of the site, discharge from the skimmer basin flows into a culvert
