Dear Mr. Danch:
On November 17, 2013, Sue Homewood of the Division of Water Resources (Division) Winston -Salem
Regional Office conducted a site visit for the purpose of permit compliance for the above referenced
project in Forsyth County, NC. Carol Von Isenburg from GeoScience was present during the inspection.
During this site visit it was determined that the stream stabilization project was not constructed in
accordance with the approval issued to you on June 14, 2013.
Accordingly, the following permit violations were noted during the Division of Water Resources
inspection and subsequent file review:.
1. 401 Certification Authorization # 13 -0494 issued on June 14, 2013 states "This approval
is for the purpose and designed described in your application. The plans and
specifications for this project are incorporated by reference as part of the Certification.
During the site visit and discussion with your engineer it was determined that corrective action
would be necessary. Corrective action shall be conducted under the supervision of the
Engineer and shall consist of removal of riprap, reduction of stream bank slope, and
restabilization with natural fiber matting upon completion, in areas identified during the site
visit on November 17, 2013. This work shall be completed within 60 days of this notification.
