Please see below. I know from past experience that we have been able to sign documents such as waste manifests on
behalf of the State Lead Program, but I do not see a specific agent letter in our contract files. If there is no such agent
letter, would you please shoot me an email response indicating that we are designated to sign the UIC Notice of Intent
for Lewis Service Station site on your behalf for me to forward on? I believe that will suffice for Shristi and DWR's
purposes and will be the most expedient solution without having to create a letter.

Shristi/Michael,
On behalf of Mark Petermann with the State Lead Program, please see the attached Notice of Intent. Jeff Albano with
RemRx had previously discussed with Michael the use of RemRx's version of the already -approved Regenesis hydrogen
peroxide product. As detailed in the NOI, we plan to introduce the anhydrous hydrogen peroxide into five treatment
wells installed within the source area of this State Lead site. Drilling of the treatment wells is currently scheduled for
Monday 11/22 and 11/23, with the deployment of the hydrogen peroxide product the following week, after the
Thanksgiving holiday. Please let us know if you have any questions of comments.
Please let me know if a hard copy/CD of this NOI needs to be sent to Raleigh. If so, I'll get that in motion on Monday.
