Subject: City of Trinity MS4 Annual Report

On March 31, 2021, City of Trinity received a notice of violation on it NPDES MS4 Permit No. NCS000502.
On August 4, 2021, Withers and Ravenel, working for City of Trinity, turned in the Draft Stormwater
Management Plan to NCDEQ. The draft has not been approved by NCDEQ as of November 18, 2021.

We currently have done a self-audit with Withers and Ravenel and they turned it in on August 4, 2021.
We are working on an Operation and Maintenance Program Plan and a Illicit Discharge Detection and
Elimination Program Plan. We do have a quick draft of each document, but they haven’t been put in a
plan format yet.
