The results detailed below represent the status for each of your certified parameters and methods to date. A blank in the "PE complete" column indicates no PE yet recorded for this calendar year. A "Pass" entry indicates your lab has satisfied the annual performance evaluation requirement for that parameter and/or method. Other information in this column indicates vendor and Study identification. A "Fail" entry requires a follow up sample to be analyzed and reported to this office within 60 days. An N/A entry represents parameter exemption from the performance evaluation requirement. A date entered beside the "Fail" entry represents the completion deadline for this PE follow up. ` I. Should a PE provider notify us of a failure on one of your certified parameters, you will be required to analyze add submit a follow up blind PE result. Failure to analyze and submit an acceptable follow up sample result within 60 days or producing a second successive unacceptable result will result in decertification for that parameter.
