Director Division of Water Quality DENR-FRO NOV 2 8 2005 ®INO One of the requirements of your North Carolina laboratory certification is analysis of a Performance Evaluation (PE) sample obtained from an accredited vendor. To date we have not received any PE results for your facility. You are reminded that an acceptable performance evaluation result must be obtained for all certified parameters which are available from the accredited vendors. Failure to submit a .passing PE result by December 31, 2005. will be considered a first miss. Failure to analyze and submit an acceptable follow up sample result within 60 days after a failed PE or producing a second successive unacceptable result may result in decertification for that parameter. Director Division of Water Quality The performance evaluation (PE) results submitted by your laboratory from the ERA DMR-QA Performance Evaluation Study have been received and evaluated. The attached table represents the status for each of your certified parameters and methods to date. A "Pass" entry indicates your lab has satisfied the annual performance evaluation requirement. Other information in this column indicates vendor and Study identification. An N/A entry represents parameter exemption from the performance evaluation requirement. If a follow up analysis is required a date will be indicated to the right.
