Dear Mr. Montebello,
This letter is a formal request by the City of Winston Salem for a reduction in monitoring frequency of
Conductivity for the Muddy Creek WWTP Permit#NC0050342.
Our Muddy Creek facility has not had any permit limit violation for Conductivity in the past three years
and there have been no criminal violations of the Clean Water Act in the previous five years. The facility
is not currently under an SOC for Conductivity nor is it listed on EPA's Quarterly Noncompliance Report.
The City of Winston Salem is requesting this reduction in monitoring frequency to reduce overall
operating costs at our facilities.
Please see the attached table showing a three-year span of results for Conductivity at our Muddy Creek
Facility.

*The minimum Conductivity on 10/22/2018 was an anomaly.The Muddy Creek facility experienced
a large rain event prior to this sampling event. This 2ample was duplicated and the results confirm.
