Dear Sarah & Graham:
In accordance with your permit renewal application received August 23, 2021, I am forwarding Permit
No. WI0500414 for the continued operation of geothermal heating/cooling water return well(s) located
at the above referenced address. Please note that this renewed permit shall become effective on
November 5, 2021 until October 31, 2026 and shall be subject to the conditions and limitations stated
therein.
Please Note:
• Per permit II.4, Well Construction General Conditions, it is required to have both influent
and effluent sampling ports. During inspection our regional staff noticed that effluent port
was missing, therefore they were unable to collect water sample prior to injection into the
return well. This needs to be repaired no later than May 5th 2022. Notification should be
sent to Raleigh Regional Office this is done.
• A sample from the influent sampling port of your geothermal well system was collected
on October 13, 2021. Analytical results will be forwarded to you at a later date.
In order to continue uninterrupted legal use of this well for the stated purpose, you should submit an
application to renew the permit 120 days prior to its expiration date. As indicated in the permit, this
permit is not transferable to any person without prior notice to, and approval by, the Director of the
Division of Water Resources. If you have any questions regarding your permit or the Underground
