Please consider the following submission of requested additional information:
1) The purpose of the unauthorized fill placement activities was to create a building pad for overnight rental units. The
7 rental units are prefabricated 8' x 12' wooden utility sheds. The prefabricated 8' x 16' shed is to be utilized as a
restroom. This unit has a plumbing system with an approved septic drainfield located on another portion of the
property. Please refer to the attached Land Use Map (G4).
2) The culvert does appear to have proper placement for aquatic organism passage. These mechanisms include: No
vertical drop at outlet; sufficient pool depth at outlet; sufficient depth; non -excessive velocity and
turbulence. Please refer to the attached photos.

On September 30, 2021, the Division of Water Resources (Division) received your application requesting a 401 Water
Quality Certification from the Division for the subject project. The Division has determine that your application is
incomplete and cannot be processed. The application is on -hold until the following information is received:
1. What is the purpose of the 0.330 acres of wetland fill? If buildings are proposed, provide a map showing the
location, dimensions, and type of any structures to be erected on the lands for use in connection with the
project purpose. [15A NCAC 02H .0502(a)(4) and (9)]
2. Is the 40-foot access road culvert properly buried to provide for aquatic organism passage? Please provide
pictures of the inlet and outlet of the culvert. [15A NCAC 02H .0506(b)(3)]

Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the
proper consideration of the application. Please provide your response by November 12, 2021. If all of the requested
information is not received, the Division will be unable to approve the application and it will be denied as incomplete.
The denial of this project will necessitate reapplication to the Division for approval, including complete application and
the appropriate fee.
