Project Description: The proposed project involves replacing Bridge No. 346 on Hunting Hills Lane over the South Fork New River (SFNR) in the Town of Boone (Watauga County) (see Figure 1 —Vicinity Map). The present structure including two traffic lanes, one bicycle lane, and one sidewalk, is approximately 60' in length and 24.5' in width. This structure will be replaced in situ with adjacent road work totaling 729.63' of TIP length. The structure will be replaced in place and in stages, as there is a closed residential neighborhood loop north of the bridge. A single lane for access across the structure will be maintained during the bridge replacement. The current posted speed limit is 20 mph; the design speed will be 35 mph using AASHTO guidelines. B. Description of Need and Purpose: The purpose of this project is to replace a deteriorating bridge structure. Bridge No. 346 was built in 1970 and is owned and operated by the Town of Boone. It consists of 2 —30' spans of Prestressed Concrete Channels on Reinforced Concrete Abutments and one Reinforced Concrete Pierwall Bent. The out-out bridge width is 24.5'. The lowest Sufficiency Rating was reported as 48 in 2008. In 2009, municipal funds were expended for bridge preservation activities including shotcreting of the superstructure (2009) and timber crutch bents to address lowered SV and TTST postings (2016). Presently, the preservation activities performed in 2009 are failing and timber crutch bents are increasing debris problems at the site. The Town of Boone requested STIP funding in 2017 to replace the bridge structure in order to maintain the safety and accessibility of school bus, garbage, snow clearing services and emergency vehicle services to the 20 existing houses, of which the bridge is the only access, should the SV and TTST postings decrease again as the bridge continues to deteriorate. C. Categorical Exclusion Action Classification: Type I(A) - Ground Disturbing Action D. Proposed Improvements: 28. Bridge rehabilitation, reconstruction, or replacement or the construction of grade separation to replace existing at-grade railroad crossings, if the actions meet the constraints in 23 CFR 771.117(e) (1-6). NOTE: The following Type I(C) Actions (NCDOT-FHWA 2019 CE Agreement, Appendix A) only require completion of Sections A through D to substantiate and document the CE classification: 1, 5, 8 (signs and pavement markings only), 11, 13, 14, 15, 16, 17, 19, and 20; or several other Type 1 Action subcategories identified in past NCDOT-FHWA CE Programmatic Agreements (see Appendix D). Pre-approval as a CE does not exempt activities from compliance with other federal environmental laws. E. Special Project Information: The proposed bridge structure is 140' in length and the out-out bridge width is 39'. The proposed span structure includes two (2) reinforced concrete abutments spanning the entire SFNR with no interior piers. Two (2), 10' travel lanes are proposed with 26' clear roadway and a 10'-5" multiuse path. Adjacent roadwork north and south of the proposed structure totals 590'. v2019.1 B-5979 Type Choose an item. No comments have been received to date. Tribal coordination letters were distributed on November 12, 2019 to the Eastern Band of Cherokee Indians (EBCI), the Catawba Indian Nation, the Cherokee Nation, Muscogee Nation, and the United Keetoowah Band of Cherokee. Comments on the proposed project were requested by January 10, 2020. Only the Catawba Indian Nation responded, requesting to be alerted if Native American artifacts or human remains are located during the ground disturbance phase of the project. Additional Documentation as Required from Section F (ONLY for questions marked `Yes'): Response to Question 8 - Federally endangered Gray bat (Myotis grisescens) and Virginia big-eared bat (Corynorhinus townsendii virginianus) both have the potential to occur in Watauga County. The USFWS has recommended surveying the bridge for any signs of bat use before demolition to ensure the take of a federally listed species does not occur. Response to Question 10 - The SFNR at the project site is listed on the most recent Final 303(d) List (2018) for the State of North Carolina. As a result, the NC Division of Water Resources has requested that erosion control measures shall use Design Standards in Sensitive Watersheds. Response to question 11 —The SFNR is within a trout watershed, as designated by USACE. Furthermore, NCWRC has identified the SFNR as supporting a wild brown trout population and has recommended a moratorium on all in water work and work within a 25' buffer of the waterway from October 15 to April 15 of any given year. Response to Question 16 - The Project does encroach on floodplains. The Bridge Survey Report has been approved by NCDOT and the MOA is being prepared for submission to the NC Floodplain Mapping Program. Therefore, the Division shall submit sealed As-built construction plans to the Hydraulics Unit upon completion of structure construction, certifying that the drainage structure(s) and roadway embankment that are located within the 100-year floodplain were built as shown in the construction plans, both horizontally and vertically. The NC Division of Water Resources has required Design Standard in Sensitive Watersheds (15A NCAC 4B.0 1 24[a]-[e] for the project. NC Wildlife Resource Commission — Eastern Hellbender Survey Coordination NCWRC staff has requested the opportunity to survey the project site for Eastern hellbender (Cryptobranchus alleganiensis) (Federal Species of Concern) during the summer of 2020, and if individuals are observed, then they may try and re-survey and relocate them to another area prior to construction. SUMMARY OF HISTORIC ARCHICTECTURE AND LANDSCAPES REVIEW Description of review activities, results, and conclusions: Review of HPO quad maps, HPO GIS information, historic designations roster, and indexes was undertaken on November 18, 2019. Based on this review, there are no existing NR, SL, LD, DE, or SS properties in the Area of Potential Effects, which is defined on the following maps. There are no properties over fifty years of age that warrant further evaluation based on a review of properties that fall within the APE. An armory building southeast of the bridge was built 1974 based on GIS tax records and is under fifty years of age. There are no National Register listed or eligible properties and no survey is required. If design plans change, additional review will be required. The Area of Potential Effects (A.P.E.) is approximately 168 meters (550 ft.) long and 18 meters (60 ft.)wide. The project is federally funded and will require federal permits. Easements will be required. Since it is a federal undertaking the Federal Highway Administration (FHwA), with the North Carolina Department of Transportation(NCDOT) acting as its agent,must consider the protection of historic and archaeological resources in the decision-making process of transportation projects. Accordingly, this archaeological project was conducted pursuant to Section 106 of the National Historic Preservation Act and the Advisory Council on Historic Preservation's Regulations for Compliance(36 CFR Part 800). The NCDOT, in consultation with the State Historic Preservation Office (HPO), must make a `reasonable and good faith effort' to identify historic properties that may be effected, and gather sufficient information to evaluate their eligibility for inclusion on the National Register of Historic Places (NRHP). This project falls within a North Carolina County in which the following federally recognized Tribe(s) have expressed an interest: Cherokee Nation of Oklahoma, Eastern Band of Cherokee Indians, the United Keetoowah Band of Cherokee Indians, Catawba Indian Nation, and the Muskogee (Creek) Nation. It is recommended that you contact each federal agency, if involved with your project, to determine their Section 106 Tribal consultation requirements. SUMMARY OF CULTURAL RESOURCES REVIEW Brief'description of review activities, results of review, and conclusions: The review included an examination of a topographic map, an aerial photograph, and listings of previously recorded sites,previous archaeological surveys, and previous environmental reviews at the Office of State Archaeology (O.S.A.). Bridge 346 is oriented northeast-southwest, but is considered east-west for this review. The topographic map (Boone) shows the A.P.E. is located in a wide river valley. The landform in the A.P.E. is a wide, level floodplain. The floodplain is depicted as cleared, indicating it may be "No ARCHAEOLOGY SUR VEYREQUIRED"form for Minor Transportation Projects as Qualified in the 2015 Programmatic Agreement. 1 of 6 DocuSign Envelope ID: B49D3F13-04D8-4E76-A81F-123F08A78A00 Project Tracking No. 19-11-0011 suitable for agriculture (well drained). There is a road at the west end of the A.P.E. Well drained floodplain landforms have a moderate to high potential for prehistoric archaeological sites. The aerial photograph shows the landuse in the A.P.E. is a mix of wooded and developed. The land on the west side of the bridge is developed. The land on the east side of the bridge is wooded. There is a driveway in the northeast quadrant. There is a house near the southeast quadrant. A review of information at the O.S.A. shows there are no previously recorded archaeological sites within the A.P.E. There are several sites recorded a short distance upstream and downstream. The A.P.E. is within an area that has been included in a previous archaeological survey(ER 00-8701). White (2001) conducted an archaeological survey for a plan to restore the South Fork New River to a more natural condition. The survey along each side of the South Fork did not identify any archaeological sites. It did find evidence of major disturbance from flooding, channelization(for flood control), and road construction. Another project that has been surveyed is located at the west end of the A.P.E. Bowden (1998) conducted an archaeological survey of the proposed "Boone Loop Phase III" project(ER 98-7544; ER 98-8536). the survey of the bike lane extends from the southwest quadrant of the A.P.E. south and then west along Winkler Creek to US 321/US 221. The survey identified one archaeological site (31 WT328) along Winkler's Creek.
