This WWTP utilizes the following treatment components: • Aeration basin • Clarifier • Chlorine contact chamber with tablet chlorination • Tablet dechlorinator • Sludge holding tank Staff Report from Mikal Willmer (ARO) noted a discrepancy between the originally issued AtC, which was originally issued for a 0.010 MGD plant, and the permitted flow on previous versions of the permit, which has been 0.020 MGD. DWR records indicate that the plant would be phased up to 0.02 MGD as the development grew, with the addition of a second treatment train, but no additional or revised AtC was ever issued. During the inspection by Mikal Willmer, the facility didn't appear to be adequately sized for a permitted flow of 0.02 MGD. The ORC conducted a volume test and determined the plant volume is 10,000 gallons, per the email from Environmental Inc (see attached). Renewal Summary: This renewal contains the following proposed changes: • Updated the Flow limit in the effluent table in Section A. (1.) to reflect the accurate capacity of the current wastewater treatment plant. • Updated the Ammonia Nitrogen (Nov 1 — Mar 31) limits in the effluent table in Section A. (1.) so that they are no greater than twice the summer (Apr 1 — Oct 31) limits, as per current permitting policy. • Updated Section A. (2.) Electronic Reporting of Discharge Monitoring Reports to reflect current federal requirements. Fact Sheet for Permit Renewal October 2021 -- NPDES Permit NC0065986 - Page 1 Compliance History (Aug 2016 — Aug 2021): Dogwood Trails Subdivision WWTP has no enforcements on record. The facility has had two NOVs during the last permit cycle; • BOD limit violation • Temperature monitoring violation Most recent inspection in 2020 shows the facility was in compliance. Comments Received: ARO provided clarification that the digester listed under the facility treatment components is actually a sludge holding tank, as documented in the 2020 ARO Staff Report by Mikal Willmer (see attached). The components list on the Supplement to Cover Sheet has been corrected. No records in our database show an additional or revised AtoC was ever issued. During my inspection, the facility did not appear adequately sized visually to be permitted for 0.02 MGD. The permittee was asked to verify or provide documentation of the true hydraulic capacity. The ORC conducted a volume test and determined the aeration basin was sized for approximately 0.01 MG. Environmental Inc. Contract firm indicated they would submit an email regarding the volume test. 3. Initial recipient of electronic NPDES information from NPDES-regulated facilities means the entity (EPA or the state authorized by EPA to implement the NPDES program) that is the designated entity for receiving electronic NPDES data [see 40 CFR 127.2(b)]. EPA plans to establish a website that will also link to the appropriate electronic reporting tool for each type of electronic submission and for each state. Any person submitting an electronic DMR using the state's eDMR system shall make the following certification [40 CFR 122.22]. NO OTHER STATEMENTS OF CERTIFICATION WILL BE ACCEPTED: "I certify, under penalty of law, that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the possibility of fines and imprisonment for knowing violations." 5. Records Retention [Supplements Section D. (6.)1 The permittee shall retain records of all Discharge Monitoring Reports, including eDMR submissions. These records or copies shall be maintained for a period of at least 3 years from the date of the report. This period may be extended by request of the Director at any time [40 CFR 122.41].
