On October 5, 2021, staff visited the home to inspect the well system. System operation has been normal. Source well water and water prior to re -injection were sampled for Metals, Total and Fecal Coliform, Nitrates, Chloride, Sulfate and Total Dissolved Solids. Sampling results did not indicate any exceedances of State groundwater standards for the parameters analyzed. Below are pictures of influent and effluent sampling locations to assist future inspectors: WQROS Staff Report Rev. 4/15/2016 Page 3 DocuSign Envelope ID: 663DC74F-75EA-47AF-AA91-AAFA3663745E water Influent sample tap from source well WQROS Staff Report Rev. 4/15/2016 Page 4 DocuSign Envelope ID: 663DC74F-75EA-47AF-AA91-AAFA3663745E i Effluent Sample Tap WQROS Staff Report Rev. 4/15/2016 Page 5
