Enclosed is your 2021 third quarter Stormwater Discharge Monitoring Report (DMR). Please note this is
the second sample submitted for this monitoring period. You are required by the permit to submit your DMR
to the Mooresville Regional DEMLR Office. A cover letter to the DWQ for transmittal of the DMR is also
attached here for your convenience. Please print the cover letter and a copy of the DMR, and sign and date
inside the gray boxes of the cover letter. Please also sign and date the certification sections on the bottom
of the page of the DMR. A copy of the signed DMR also must also be electronically uploaded to this link;
https://edocs.deg.nc.gov/Forms/SW-DMR This link is also available on the DMR form and in the email that
contained this DMR. Please fill out the date the form was uploaded at the top of the page before submittal.
The original signed cover letter and DMR should be sent via certified mail with a return receipt or via Fed -
Ex to the Mooresville Regional DEMLR Office at the address shown on the cover letter. You should make
copies of the letter and form and file them in the monitoring section of your Storm Water Pollution Prevention
Plan (SPPP). The original signed cover letter and the DMR should be sent via certified mail with a return
receipt or via Fed -Ex to the DWQ at the address shown on the cover letter by November 161" 2021

2021 Third Quarter Stormwater Discharge Monitoring Report
Dear Sir/Madam:
Please find attached the 2021 Third Quarter Stormwater Discharge Monitoring Report (DMR)
for the LKQ Full -Service facility located in Salisbury, North Carolina. This submittal covers the
reporting period July 31 s' to September 30'h, 2021. A sample was collected previously in July
2021. However, COD was not analyzed. Therefore, an additional sample was collected so that
COD could analyzed and submitted for the 2021 third quarter.
A copy of the DMR was also uploaded electronically as required. Please call me at (912) 313-
