This letter is in reference to your permit major modification request received August 23, 2021.
In a letter dated August 27, 2021, Lauren Raup-Plummer of the Water Quality Permitting Section's
Non -Discharge Branch asked you to provide additional information (copy of letter attached) no later than
the close of business on September 27, 2021. A two -week extension of this deadline was requested, and
granted, extending this date to October 11, 2021. As of today, this information has not been received,
therefore, the Division of Water Resources must return your application as incomplete in accordance with
15A NCAC 02T .0107(e)(2).
Please be advised that construction and/or operation of the subject facilities without a valid
permit is a violation of G.S. 143-215.1, and may subject you to appropriate enforcement actions in
accordance with G.S. 143-215.6A-6C. Civil penalties of up to $25,000 per day per violation may be
assessed for failure to secure a valid permit.
When the requested additional information has been obtained, a new application package and
appropriate permit processing fee may be submitted.

Division of Water Resources' Central and Regional staff has reviewed the application package
received August 23, 2021. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on September
27, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A and B, or failure to provide the additional information on or before the above requested date
may result in your application being returned as incomplete.

revised and/or additional documentation shall be signed, sealed and dated (where needed), with an

A. Overall Submittal Package:
On August 23, 2021 a major modification application package was submitted to the Non -Discharge
Branch for the Carolina Malt House, Inc. wastewater irrigation system (WQ0039181). Upon
cursory review by our central office and regional office staff, it appears that many of the required
documents were not included in the submittal, including the application form (WWIS 06-16). The
referenced application form includes a checklist, which may assist in organizing the resubmitted
major modification package: Cover Letter/Executive Summary, Application Fee, Application Form
WWIS 06-16, Property Ownership Documentation, Soil Evaluation, Agronomist Evaluation,
Hydrogeologic Report, Water Balance, Engineering Plans, Specifications, Engineering
Calculations, Site Map, Power Reliability Plan, Operation & Maintenance Plan, Residuals
Management Plan, and Additional Documentation (i.e. Certificate of Public Convenience and
Necessity, Existing Permits, Final Environmental Document, Floodway Regulation Compliance,
Operational Agreements, Threatened or Endangered Aquatic Species Documentation, and
Wastewater Chemical Analysis). Depending on the project type and scope, some of these review
areas may not apply and other specific documentation may be necessary that is not listed here.
These review areas are outlined within the most recent version of WWIS 06-16 for wastewater
irrigation systems to greater detail, please review and organize your submittal accordingly for our
review.
B. Application Fee:
I. The permit application fee for a major modification for flowrates greater than 10,000 gpd is
$395.00. The Non -Discharge Branch received a check for $245.00 (the amount for modifications
for flowrates less than 10,000 gpd), and a remaining balance of $150.00 remains for the permit
application fee. Please submit the remainder as a check, a money order, or as an electronic funds
transfer with the ePayments system.
