In accordance with your Notice of Intent (NOI) to discharge, the Division forwards herewith a
Certificate of Coverage under the subject NPDES general permit. We issue this certificate pursuant to
requirements of North Carolina General Statue 143-215.1 and the Memorandum of Agreement
between North Carolina and the US Environmental Protection agency dated October 15, 2007 (or as
subsequently amended).
The following information is included with your permit package:
• A copy of the Certificate of Coverage for your treatment facility
• A copy of General Wastewater Discharge Permit NCG530000
• A copy of a Technical Bulletin for General Wastewater Discharge Permit NCG530000
Please review the technical information, the terms, and conditions in the permit package to assure
your knowledge and understanding of compliance requirements. The Division understands that this
permit covers an existing discharge. If you plan to alter your discharge conditions, please contact Tim
Heim of the Asheville Regional Office (ARO), phone [phone], to discuss how any change
might impact this permit. Regional Office staff is available 8:00 a.m. to 5:00 p.m., Monday through
Friday, excluding State holidays.
If any parts, measurement frequencies or sampling requirements contained in this general permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, the certificate of coverage shall be final and
binding.

This Certificate of Coverage is not transferable except after notice to the Division of Water Quality.
The Division may require modification, or revocation and reissuance of the Certificate of Coverage.
This permit does not affect your legal obligation to obtain other permits required by the Division of
Water Quality, the Division of Land Resources, the Coastal Area Management Act, or any other
Federal or Local government.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge wastewater from a facility located at
Lassonde Pappas
125 Industrial Park Road, Hendersonville
Henderson County
to receiving waters designated as a unnamed tributary to Mud Creek [stream segment 6-55, a Class C
waterbody within Hydrological Unit 04-03-02 [HUC: 0601010503] of the French Broad River Basin, in
accordance with the effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, III and IV of General Permit NCG500000 as attached.
This certificate of coverage shall become effective November 1, 2021
This Certificate of Coverage shall remain in effect until expiration of the General Permit.
