Dear David Keith Davenport:
In accordance with your application for a discharge permit received on September 9, 2 02 1,
we are forwarding herewith the subject certificate of coverage (COC) to discharge under
the subject state - NPDES general permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the US Environmental Protection Agency dated
October 15, 2007 (or as subsequently amended).
In order to comply with the conditions of NCG020000 to which you are now subject, you
are required to do the following:
➢ Implement a SWPPP before beginning operations.
➢ Download and print a copy of the NCG020000 Permit from the General Permits page on
the NPDES Industrial Stormwater Program web site, which can be reached by typing
"deq.nc.gov/SW-industrial" into your browser and hitting enter.
➢ When you receive an invoice for your annual permit fee each year, pay it promptly.
Electronic payment is convenient and quick, you can access the ePayment web site by
typing "deq.nc.gov/SW-ePayments" into your browser and hitting enter.
➢ If you have not already enrolled in electronic Data Monitoring Reports (eDMR), view
the six step process and complete the registration by typing "deq..nc.gov/SW-eDMR"
into your browser and hitting enter.
➢ Keep information about your facility name, ownership, legally responsible person,
address, and stormwater outfalls up to date. See the Industrial Stormwater Program
web site for instructions.

➢ Acquire an approved Operation and Monitoring Plan before mine dewatering
discharges occur at your facility due the potential impact to adjacent wetlands. An
Operation and Monitoring Plan for your facility must be submitted to
the Washington Regional Office within 60 days of your receipt of this
permit. Contact Thom Edgerton at the Washington Regional Office at (252) 948-
3963 for more information about the approval process.
➢ Note that mine dewatering wastewater includes wet pit overflows that are caused
solely by direct rainfall and groundwater seepage and thus are subject to the limits of
this permit.
➢ Determine if an additional State Stormwater Permit is required per coastal stormwater
management rules (1A NCAC 21-1.1000). Contact Thom Edgerton at the Washington
Regional office at (252) 948-3963 to determine if this is required for your facility.
➢ Determine if an additional Post -Construction Stormwater Permit is required for your
facility. Please review guidance at https:Ildeq.nc.gov/about/divisions/energy-mineral-
land- resources/ energy- mineral -land-rules/stormwater-pro gram/post-construction.
Please take notice that this certificate of coverage is not transferable except upon approval
from the Division of Energy, Mineral, and Land Resources (Division). The Division may
require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Resources, the Coastal Area Management Act, or any
other federal or local government permit that may be required.
