4. Project Description and History
4a.Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:
Bridge is in need of replacement(Site 1),as well as a nearby 72"x 49'CMP(Site 2).Surrounding land use is rural, with a mix of cattle/pastures and woodlands.

0
4g.List the total estimated linear feet of all existing streams on the property:

4h.Explain the purpose of the proposed project:
State project 17BR11.R.152 will consist of replacing the structurally deficient NCDOT bridge#20005 over Little River with a new bridge,to improve safety(Site 1).We will also replace and
upgrade a perched 72"x 49'long round CMP on a UT of Little river with a 87"W x 63"T x 54'long CMP(Site 2).This new structure will be buried 1'to improve aquatic passage.
41.Describe the overall project in detail, including indirect impacts and the type of equipment to be used:"
The proposed bridge is a 1-span(1@70')24"PCBB with 4'caps with an out-to-out deck width of 30'which will replace the existing 1-span(1@40').Roadway fill slopes will be 2:1 or flatter.
All fill slopes will be grassed to encourage a diffuse flow pattern and passive stormwater treatment wherever rip rap is not called for in the plans.Storm water runoff from the bridge and
approach slabs will be collected by a traffic bearing grated inlet and released onto a rip rap pad and allowed to sheet flow over vegetation towards the river.
5. Jurisdictional Determinations
5a.Have the wetlands or streams been delineated on the property or proposed impact areas?*

E. Impact Justification and Mitigation u
1.Avoidance and Minimization
la.Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
The existing bridge will remain in place during construction, preventing the need for a detour. All work will be done in the dry,and no direct discharge of
sediment will occur into Little River or the UT on the project site. The erosion control measures have been improved to fulfill the design standards for
sensitive watershed requirements.
I b.Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
All Best Management Practices will be adhered to.The erosion control measures have been improved to fulfill the design standards for sensitive
watershed requirements.
2.Compensatory Mitigation for Impacts to Waters of the U.S.or Waters of the State
2a.Does the project require Compensatory Mitigation for impacts to Waters of the U.S.or Waters of the State?
Yes No
2b.If this project DOES NOT require Compensatory Mitigation, explain why:
There is 5'of new fill, from the extension of the existing 49'CMP to a 54'CMP.This increase in length will improve the buffer width between the roadway and pipe inlet/outlet,and help
improve the safety of motorist by providing room for guardrail around the new CMP.NCDOT has not purchased mitigation for this 5'of new fill, but will do so if USACE requires it.
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
la.Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
Yes No
If no,explain why:
2. Stormwater Management Plan
2a.Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?
Yes No
Comments:
NPDES forms will be filled out weekly, and after any 1"rainfall.All erosion control measures will employ design standards for sensitive waters.
G. Supplementary Information
