On October 1, 2013, the Division of Water Resources (Division) received your application dated
October 1, 2013, requesting a 401 Certification from the Division for the subject project. The
following information was omitted and is necessary for the processing of an application:
Page 8 of 10 — Stormwater Management and Diffuse Flow Plan. The application indicates
that this project is a future hospital/medical site development. Item #2 (overall percent
impervious of project) must be completed for the future development project and all other
stormwater questions should be answered based on the final development plans. A
stormwater management plan for the entire project area must be submitted according to
Attachment A of the General Certification #3886.

Pursuant to Title 15A NCAC 02H .0507(e), the Division is unable to process your application
and your application is hereby returned. You will need to reapply to the Division for approval
including a complete application package and the appropriate fee.
Please be aware that you have no authorization under Section 401 of the Clean Water Act for
this activity and any work done within waters of the state and/or the protected riparian buffer
may be a violation of North Carolina General Statutes and Administrative Code.
