On September 16, 2021, the Division of Water Resources (Division) received your application requesting a 401 Water
Quality Certification from the Division for the subject project. The Division has determined that your application is
incomplete and cannot be processed. The application is on -hold until the following information is received:
1. The Pre -Construction Notification (PCN) lists the applicant as JVM Investments and Leasing, LLC. Title 15A of the
North Carolina Administrative Code (NCAC) 02H .0502(a)(3) requires an applicant that is a corporation be
registered with the NC Secretary of State's office. I was unable to find the applicant in a search conducted on
the NC Secretary of State's website. Please provide documentation that the applicant is a registered
corporation with the NC Secretary of State. Alternatively, John Meshad, the individual, may be the applicant.
Regardless of who the applicant ultimately is, the Division will also need an agent authorization form signed by
the applicant granting you signature authority. A sample agent authorization form may be found here

Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the
proper consideration of the application. Please provide your response by November 4, 2021. If all of the requested
information is not received, the Division will be unable to approve the application and it will be denied as incomplete.
The denial of this project will necessitate reapplication to the Division for approval, including complete application and
the appropriate fee.
