Attached is the Initial Interest Form for Express Review Program. I am requesting a buffer
determination for the attached site. The NCRS soils map indicates a pond used to be located on the
site. There is no longer a pond on the property. I have not been able to determine when the pond was
filled, but aerial photos as early as 2000 do not show the pond. The USGS topographical map does
not indicate a pond at the site. If you have any questions, please advise.

Please provide a brief description of this project (attach site plan if available):
Construction of a commercial medical office complex. NRCS Soils Map indicate a pond was located on the
property. The property has since been altered after publication of the latest soil survey (1994) and no lon er has a
pond. Aerial photographs from 2001 indicate that the pond no longer existed Property has a stormwater conveyance
channel alone the eastern property line. The conveyance enters a culvert under Berkshire Rd and discharges into UT
to Buffalo Creek as indicated by the USGS topographical maw

Site is addressed as 910 Berkshire Road, Smithfield, Johnston Count!, NC. From Raleigh, take US Hwy 70 E to
Selma/I-95. Turn right on Hwy 301 (Brightleaf Blvd). Site is located approximately 0.3 miles past intersection with
Canterbury Rd on right side of Hwy 301. Sign is posted desi ng atin~ future location of Smithfield Heart & Vascular
Associates.
