On February 13, 2012, Mandy Hall of the Raleigh Regional Office visited your single-family residence
(SFR) wastewater treatment system to evaluate compliance with the above permit to discharge
wastewater. Your follow-up phone call was appreciated. The checked box below show what condition
were noted at your facility:
® In compliance. You are reminded to regularly maintain the chlorine disinfection system and
have the effluent sampled once a year. Even if your facility does not discharge have the septic
tank pumped out every 3 to 5 years. Your good record of operation and meeting the permit
requirements is highly commended.

Visited residence and left form for telephone follow-up. Saw no issues when doing the inspection. System
is on the left side of the home.
Ms. Nelson returned phone call and indicated she is using bio-sanitizer and bio-neutrilizer tablets for
chlorination and de -chlorination. Septic Tank was last pumped in June 2011.
